摘要
目的 :回顾性分析沙库巴曲缬沙坦治疗扩张型心肌病(DCM)和缺血性心肌病(ICM)所致射血分数降低型心力衰竭(HFrEF)患者的临床疗效。方法 :收集2019年9月1日~2021年8月31日在长沙市第四医院心血管内科住院并首次服用沙库巴曲缬沙坦治疗的HFrEF患者,DCM组患者56例,ICM组患者57例,其中有完整心脏结构指标的患者共90例,DCM组与ICM组各45例。观察最大左室射血分数值(LVEF)、最小左室舒张末内径值(LVEDD)、最小左房舒张末内径值(LAEDD)、1年内再入院率和死亡终点事件。结果 :两组患者治疗3个月后LVEF高于治疗前,LVEDD、LAEDD低于治疗前,差异具有统计学意义。两组患者LVEF上升幅度、LVEDD与LAEDD下降幅度、1年内再入院率和死亡终点事件无明显差异。结论 :沙库巴曲缬沙坦可逆转DCM与ICM所致射血分数降低型心力衰竭患者心房及心室重构,改善心功能。
Objective To explore the efficacy of Sacubitril Valsartan on HFrEF caused by ICM and DCM. Methods A total of 56 patients with DCM and 57 patients with ICM who were hospitalized in the Department of Cardiovascular of the Fourth Hospital of Changsha from September 1, 2019 to August 31, 2021 and were treated with Sacubitril Valsartan for the first time,and 90 patients with intact echocardiography indicators, 45 each in the DCM and ICM groups. The observation indexes included maximum Left Ventricular Ejection Fraction(LVEF), minimum Left Ventricular End-diastolic Diameter(LVEDD) and Left Atrial End-diastolic Diameter(LAEDD), the readmission rate and endpoint event of death within one year. Results LVEF increased, LVEDD and LAEDD decreased after taking Sacubitril Valsartan for at least 3 months, with statistically significant differences before and after treatment. there was no statistical difference between the DCM and ICM groups in terms of the increase in LVEF, decrease in LVEDD and LAEDD, the readmission rate and endpoint event of death within one year. Conclusion Sacubitril Valsartan can effectively reverse atrial and ventricular remodeling, and improve heart function.
