摘要
目的探讨儿童颈部皮样囊肿及表皮样囊肿的临床特征,为疾病的诊治提供帮助。方法回顾性纳入经手术治疗且术后病理学确诊为皮样囊肿及表皮样囊肿的患儿68例,分析患儿的性别构成、发病年龄、病灶分布及病理特征。结果皮样囊肿和表皮样囊肿的性别构成差异无统计学意义(P>0.05)。皮样囊肿和表皮样囊肿的主要发病年龄段均为1岁<年龄≤5岁,其次为出生<年龄≤1岁(P<0.05);两者均主要分布在颈中线舌骨水平,其次在舌骨下水平,囊肿分布差异有统计学意义(P<0.05)。在组织病理学检查中,皮样囊肿囊壁含皮肤附属器,表皮样囊肿囊壁则不包含。结论颈部皮样囊肿和表皮样囊肿具有无性别差异、临床表现相同、主要发病年龄一致、病变外观和发病部位相似的特征,组织病理学检查是最可靠的区分方法。
Objective Explore the clinical features of cervical dermoid cyst and epidermoid cyst in children,and to provide help for the diagnosis and treatment of the disease.Method A total of 68 children with dermoid cyst and epidermoid cyst confirmed by postoperative pathology after surgical treatment were retrospectively included.The gender composition,age of onset,distribution of lesions and pathological characteristics of the children were analyzed.Result There was no significant difference in sex composition between dermoid cyst and epidermoid cyst(P>0.05).The main onset age of dermoid cyst and epidermoid cyst was 1 year<age≤5 years old,followed by birth<age≤1 years old(P<0.05).Both of them were mainly distributed at the level of midline cervical hyoid bone,followed by the level of Subhyoid bone.The difference of cyst distribution was statistically significant(P<0.05).Histopathological examination showed that dermoid cyst wall contained skin appendages,while epidermoid cyst wall did not.Conclusion Cervical dermoid cysts and epidermoid cysts have the characteristics of asexual difference,the same clinical manifestations,the same age of onset,similar appearance and location of lesions.Histopathological examination is the most reliable method to distinguish them.
