摘要
目的探究人类免疫缺陷病毒(HIV)/乙型肝炎病毒(HBV)共感染患者免疫功能与肝组织纤维化的相关性。方法选取该院60例慢性HIV/HBV共感染患者作为研究组,另选同期60例单纯慢性乙型肝炎(CHB)患者作为对照A组,60例单纯艾滋病患者作为对照B组,比较3组一般资料、血常规指标、肝功能指标、免疫学指标、炎症因子、肝纤维化指标水平,分析免疫学指标与HIV/HBV共感染患者肝组织炎症和肝组织纤维化程度的相关性,并分析免疫功能缺陷状态与肝组织炎症和纤维化程度的关联性。结果研究组外周血总胆红素(TBIL)、间接胆红素(IBIL)、丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、γ-谷氨酰基转移酶(GGT)、凝血酶原时间(PT)、CD8^(+)、血清免疫球蛋白(Ig)G、IgA、干扰素-γ(IFN-γ)、白细胞介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)、透明质酸(HA)、层黏连蛋白(LN)、Ⅲ型前胶原(PCⅢ)、Ⅳ胶原蛋白(Ⅳ-C)水平均高于对照A组、对照B组,清蛋白与球蛋白比值(A/G)、凝血酶原活动度(PTA)、CD3^(+)、CD4^(+)、自然杀伤(NK)细胞水平低于对照A组、对照B组,差异有统计学意义(P<0.05);对照A组外周血CD3^(+)、CD4^(+)、NK细胞、HA、LN、PCⅢ、Ⅳ-C水平均高于对照B组,CD8^(+)、IgG、IgA、IFN-γ、IL-4均低于对照B组,差异有统计学意义(P<0.05);外周血CD8^(+)、IgM、IgG、IgA、IFN-γ、IL-4水平与HIV/HBV共感染患者肝组织炎症和肝组织纤维化程度呈正相关,CD3^(+)、CD4^(+)、NK细胞水平与HIV/HBV共感染患者肝组织炎症和肝组织纤维化程度呈负相关(P<0.05);HIV/HBV共感染患者免疫功能缺陷状态与肝组织炎症和纤维化程度呈正相关(P<0.05)。结论HIV/HBV共感染患者免疫功能与肝组织炎症和纤维化程度密切相关,检测患者免疫功能指标有助于评估HIV/HBV共感染患者肝组织炎症和纤维化程度。
Objective To explore the correlation between immune function and liver fibrosis in patients with human immunodeficiency virus(HIV)/hepatitis B virus(HBV)coinfection.Methods A total of 60 patients with chronic HIV/HBV coinfection admitted in the hospital were selected as the study group,60 patients with simple chronic hepatitis B(CHB)in the same period were selected as the control group A,and 60 patients with simple acquired immune deficiency syndrome were selected as the control group B.The general data,blood routine indicators,liver function indicators,immunological indicators,inflammatory factors,and liver fibrosis indicators in the three groups were compared,the correlation between immunological indicators and the degree of liver tissue inflammation and liver fibrosis in patients with HIV/HBV coinfection was analyzed,and the correlation between the status of immune function deficiency and the degree of liver tissue inflammation and fibrosis was analyzed.Results Total bilirubin(TBIL),indirect bilirubin(IBIL),alanine aminotransferase(ALT),aspartate aminotransferase(AST),γ-Glutamyltransferase(GGT),coagulation index prothrombin time(PT),CD8^(+),serum immunoglobulin(Ig)G,IgA,interferon-γ(IFN-γ),interleukin-4(IL-4),tumor necrosis factor-α(TNF-α),hyaluronic acid(HA),laminin(LN),typeⅢprocollagen(PCⅢ),and typeⅣcollagen(Ⅳ-C)levels in the study group were higher than those in control group A and control group B,and albumin to globulin ratio(A/G),prothrombin activity(PTA),CD3^(+),CD4^(+),and natural killer(NK)cells levels were lower than those in control group A and control group B,and the differences were statistically significant(P<0.05).The levels of CD3^(+),CD4^(+),NK cells,HA,LN,PCⅢ,Ⅳ-C in the control group A were higher than those in the control group B,the levels of CD8^(+),IgG,IgA,IFN-γand IL-4 were lower than those in the control group B,and the differences were statistically significant(P<0.05).Peripheral blood CD8^(+),IgM,IgG,IgA,IFN-γand IL-4 levels were positively correlated with the degree
