摘要
目的探讨连续收肌管阻滞联合坐骨神经阻滞对膝关节置换术后疼痛、应激反应及关节功能的影响。方法选取2018年1月至2021年1月驻马店市第一人民医院收治的膝关节置换术患者88例,根据麻醉方式不同分为对照组与观察组。对照组应用连续收肌管阻滞麻醉,观察组应用连续收肌管阻滞联合坐骨神经阻滞,比较两组术后数字评价量表(NRS)、心率(HR)、平均动脉压(MAP)与关节活动度。结果观察组术后24 h与48 h的NRS评分分别为(3.80±0.72)分、(2.60±0.50)分,低于对照组,组间对比差异有统计学意义(P<0.05);观察组术后24 h与48 h的HR与MAP低于对照组,组间对比差异有统计学意义(P<0.05);观察组术后3d的关节活动度(88.54±4.78)°,高于对照组[(83.62±5.30)°],有统计学意义(P<0.05);两组不良反应发生率差异无统计学意义(P>0.05)。结论连续收肌管阻滞联合坐骨神经阻滞可减轻膝关节置换术患者的术后疼痛度,缓解应激反应,并加快关节功能恢复。
Objective To investigate the effects of continuous adductor canal block combined with sciatic nerve block on pain,stress response and joint function after knee replacement.Methods A total of 88 patients with knee replacement in our hospital from January 2018 to January 2021 were selected and divided into control group and observation group according to different anesthesia methods.The control group received continuous adductor tube block anesthesia,and the observation group received after operation continuous adductor tube block combined with sciatic nerve block.NRS,HR,MAP and range of motion were compared between the two groups.Results The NRS scores of the observation group were(3.80±0.72)points and(2.60±0.50)points respectively at 24 h and 48 h after surgery,which were lower than those of the control group(P<0.05).HR and MAP in observation group were lower than those in control group at 24 h and 48 h after operation(P<0.05).The range of motion(88.54±4.78)in the observation group was higher than that in the control group(83.62±5.30),with statistical significance(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Continuous adductor canal block combined with sciatic nerve block can reduce postoperative pain,relieve stress response and speed up the recovery of joint function in patients undergoing knee replacement.
