摘要
目的探讨血清C反应蛋白(CRP)、降钙素原(PCT)及白细胞计数(WBC)检测对小儿支气管炎病情及复发的评估价值。方法选取2018年10月至2020年4月于雅安市雨城区人民医院收治的83例小儿支气管炎患儿为研究对象,并将其设为研究组,根据病情严重程度将其分为重症组(n=47)和轻症组(n=36)两个亚组;另选取30例同期体检健康儿童设为对照组。比较研究组各亚组间及与对照组小儿CRP、PCT和WBC水平的差异,分析上述指标对小儿重症支气管炎的诊断价值和疾病复发的危险因素。结果研究组CRP、PCT和WBC均明显高于对照组(P<0.05),重症组CRP、PCT和WBC均明显高于轻症组(P<0.05);CRP、PCT和WBC诊断小儿支气管炎重症的ROC曲线下面积分别为0.767、0.887、0.851;Logistic回归性分析结果显示,疾病严重程度、治疗疗程、合并过敏性鼻炎、CRP、PCT和WBC均为影响支气管炎患儿疾病复发的独立危险因素(P<0.05)。结论血清CRP、PCT和WBC对评估小儿支气管炎病情及复发均有意义,可作为预测病情严重程度和预后的有效指标。
Objective To investigate the value of serum C-reactive protein(CRP),procalcitonin(PCT)and white blood cell count(WBC)in the evaluation of bronchitis in children.Methods A total of 83 children with bronchitis who were treated in our hospital from October 2018 to April 2020 were selected as the study group and divided into two subgroups according to the severity of their disease:severe group(n=47)and mild group(n=36).Another 30 healthy subjects were selected as the control group.The levels of CRP,PCT and WBC in children of the study group and the control group were compared,and the diagnostic value of the above indexes for severe bronchitis in children and the risk factors for disease recurrence were analyzed.Results CRP,PCT and WBC in study group were significantly higher than those in control group(P<0.05),and CRP,PCT and WBC in severe group were significantly higher than those in mild group(P<0.05).The areas under ROC curve for CRP,PCT and WBC in diagnosing severe bronchitis in children were 0.767,0.887 and 0.851,respectively.Logistic regression analysis showed that disease severity,treatment course,allergic rhinitis,CRP,PCT and WBC were independent risk factors for disease recurrence in children with bronchitis(P<0.05).Conclusion Serum CRP,PCT and WBC have significance in evaluating the disease and recurrence of bronchitis in children,and can be used as an effective indicator to predict the severity and prognosis of the disease.
