摘要
目的检测子痫前期患者的外周血血清中sTWEAK的表达水平,并分析其临床意义。方法选取2017年5月至2019年3月在上海交通大学附属新华医院崇明分院产检及入院分娩的轻度子痫前期、重度子痫前期及正常孕妇共90例,采用酶联免疫吸附试验(ELISA)方法检测外周血sTWEAK的表达水平。结果与正常孕妇组相比,重度子痫前期组及轻度子痫前期组的体重指数明显升高(P<0.01),重度子痫前期组的糖化血红蛋白明显升高(P<0.01)。与重度子痫前期组相比,轻度子痫前期组的糖化血红蛋白明显降低(P<0.01)。重度子痫前期组、轻度子痫前期组及正常孕妇组的sTWEAK表达水平3组比较,差异均有统计学意义(P<0.01)。与血糖正常组相比,血糖异常组孕妇sTWEAK的表达水平明显降低(P<0.01)。与BMI<24组孕妇相比,BMI≥24组的孕妇sTWEAK的表达水平明显降低(P<0.01)。所有孕妇sTWEAK的表达水平与体重指数呈负相关性(rs=-0.339,P=0.001),与糖化血红蛋白呈负相关性(rs=-0.251,P=0.017)。结论孕晚期sTWEAK的表达与子痫前期的发生及进展相关,其表达水平可能对预测子痫前期患者是否存在糖尿病或超重等代谢异常具有一定的作用。
Objective To explore the expression level and clinical implications of serum soluble tumor necrosis factor—like weak inducer of apoptosis(sTWEAK)in preeclampsia patients.Methods 90 patients of mild preeclampsia,severe preeclampsia and normal pregnant women were selected from Xinhua Hospital Chongming Branch,School of Medicine,Shanghai Jiao Tong University.These patients were admitted to the hospital during the period from May 2017 to March 2019.The levels of serum sTWEAK were detected by Enzyme-linked immunosorbent assay(ELISA).Results Compared to the normal pregnant group,the body?mass?index(BMI)of the severe preeclampsia group and the mild preeclampsia group were significantly increased(P<0.01),and the glycosylated?hemoglobin(HbA1 c)in the severe preeclampsia group were significantly increased(P<0.01).Compared to the severe preeclampsia group,the HbA1 c in the mild preeclampsia group were significantly reduced(P<0.01).sTWEAK expression levels of severe preeclampsia group,mild preeclampsia group and normal pregnant women group showed statistically significant differences(P<0.01).Compared to the normal group,the expression levels of sTWEAK of pregnant women in the group with abnormal blood glucose were significantly decreased(P<0.01).Compared to the group with BMI<24,the expression levels of sTWEAK were significantly decreased in the group with BMI≥24(P<0.01).In all pregnant women,the expression levels of sTWEAK were negatively correlated with BMI(rs=-0.339,P=0.001)and HbA1 c(rs=-0.251,P=0.017).Conclusion The expression of sTWEAK in late pregnancy is related to the occurrence and progress of preeclampsia,and its expression level may play a certain role in predicting whether preeclampsia patients have diabetes mellitus,overweight or other metabolic abnormalities.
