摘要
目的研究COX-2和CEA在大肠癌中的表达及其与临床病理特征之间的关联。方法随机选取大连市中心医院病理科于2017年7月至2019年7月间经手术切除的大肠癌石蜡切块50例,选取经消化内镜活检的大肠腺瘤组织50例,选取大肠癌远端部位距肿瘤边缘15cm以外正常大肠黏膜组织50例,采用免疫组织化学检测上述组织中COX-2和CEA的表达。结果(1)大肠癌和大肠腺瘤中COX2和CEA的表达均明显高于正常大肠黏膜组织,大肠腺瘤中COX2和CEA的表达均明显高于正常大肠黏膜组织,差异有统计学意义(P<0.05);(2)COX-2和CEA的表达均与大肠癌分化程度、淋巴结转移和Dukes’分期程度具有相关性(P<0.05),COX-2和CEA的表达与患者的年龄、性别和肿瘤发生部位均无明显相关性(P>0.05);(3)大肠癌组织中COX-2和CEA的表达水平呈正相关。结论COX-2和CEA在大肠癌中表达呈正相关性升高。且与肿瘤分化程度、淋巴结转移和Dukes分期相关联,而与患者的年龄、性别和肿瘤发生部位均无明显相关性。COX-2和CEA可以作为大肠癌的病理诊断标记物。
Objective To study the expression of COX-2 and CEA in colorectal cancer and their correlation with clinicopathological features.Methods 50 cases of paraffin-cut colorectal cancer section that were surgically removed from July 2017 to July 2019 in the Department of Pathology of Dalian Central Hospital were randomly selected,50 cases of colorectal adenoma tissue that were biopsied by digestive endoscopy were selected,and 50 cases of normal colorectal mucosal tissue that was 15 cm away from the distal part of colorectal cancer were selected.The expression of COX-2 and CEA was detected by immunohistochemistry.Results(1)The expressions of COX2 and CEA in colorectal cancer and colorectal adenoma were significantly higher than those in normal colon mucosa,and the expressions of COX2 and CEA in colorectal adenoma were significantly higher than those in normal colon mucosa,with statistically significant differences(P<0.05);(2)The expression of COX-2 andCEA was correlated with the degree of differentiation,lymph node metastasis and Dukes’stage of colorectal cancer(P<0.05),there was no significant correlation between the expression of COX-2 and CEA and the age,sex and tumor site of patients(P>0.05);(3)The expression levels of COX-2 and CEA in colorectal cancer tissues were positively correlated.Conclusion The expression of COX-2 and CEA was positively correlated in colorectal cancer.It was correlated with tumor differentiation,lymph node metastasis and Dukes stage,but had no significant correlation with age,sex and tumor site.Cox-2 and CEA can be used as pathological diagnostic markers for colorectal cancer.
