摘要
目的研究血清总胆红素(TBil)和氧化低密度脂蛋白(ox-LDL)水平与经皮冠状动脉(下称冠脉)介入(PCI)后支架内再狭窄(ISR)的关系。方法选择2018年1月至2020年6月在新昌县人民医院复查冠状动脉造影的冠心病患者共178例,所有患者均行PCI,根据术后是否发生ISR分为ISR组(55例)和无ISR组(123例)。比较两组患者的一般资料,检测两组患者糖化血红蛋白、TBil、ox-LDL水平,记录两组患者PCI术前冠脉分叉病变、弥漫性病变和CTO病变、植入支架数量和长度。采用多因素logistic回归分析PCI术后发生ISR的危险因素,采用ROC曲线分析TBIL、ox-LDL对ISR的预测价值。结果两组患者脉压、吸烟、糖化血红蛋白、TBil和ox-LDL水平及复杂冠脉病变、植入支架长度比较,差异均有统计学意义(均P<0.05)。多因素logistic回归分析显示,吸烟、糖化血红蛋白、TBil和ox-LDL水平均是冠心病冠状动脉植入术后发生ISR的危险因素(均P<0.05)。ROC曲线分析结果显示,TBil的AUC:0.696,95%CI:0.616~0.776,ox-LDL的AUC:0.943,95%CI:0.909~0.976。结论血清TBil和ox-LDL水平与PCI术后ISR均存在相关性,ox-LDL对ISR有较高的预测价值。
Objective To investigate the relationship between serum total bilirubin(TBil)and oxidized low density lipoprotein(ox-LDL)levels and in-stent restenosis(ISR)after coronary stent implantation.Methods A total of 178 patients with coronary stent implantation underwent coronary artery angiography reexamination in Xinchang County People's Hospital from January 2018 to June 2020.ISR was observed in 55 cases(ISR group)and not observed in 123 cases(no ISR group).The general data,glycated hemoglobin,TBil,and ox-LDL levels,preoperative coronary bifurcation disease,diffuse coronary disease,chronic total occlusion,the number of stent and stent length were compared between the two groups.Multivariate logistic regression was used to analyze the risk factors for ISR after PCI.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of TBIL and ox-LDL on ISR.Results Pulse pressure,the number of cigarettes smoked,glycated hemoglobin,TBil and ox-LDL levels,complex coronary lesions,stent length were significantly different between the two groups(all P<0.05).Multivariate logistic regression analysis showed that the number of cigarettes smoked,glycated hemoglobin,TBil and ox-LDL levels were the risk factors for the development of ISR after coronary stent implantation(all P<0.05).ROC curve analysis showed that the AUC of TBil was 0.696(95%CI:0.616-0.776),the AUC of ox-LDL was 0.943(95%CI:0.909-0.976).Conclusion Serum TBil and ox-LDL are related to ISR after coronary stent implantation.ox-LDL has higher predictive value for ISR.
