SISI联合TEOAE在噪声性听力损失筛查中的应用

摘要 目的探讨短增量敏感指数(SISI)测试及瞬态诱发耳声发射(TEOAE)在噪声性听力损失筛查中的意义。方法50例纯音听阈正常并有噪声暴露史的受试者作为噪声组,另外50例未接受噪声暴露的纯音听阈正常者作为对照组,两组平均年龄分别为19.58±1.03、19.90±1.20岁。两组分别行气导纯音听阈、SISI、TEOAE检测,分析两组间的结果差异。结果噪声组和对照组0.25~8 kHz气导纯音听阈均小于20 dB HL,差异无统计学意义(P>0.05)。噪声组各频率SISI均高于对照组,差异有显著统计学意义(P<0.01)。噪声组TEOAE波形总体相关率为92.32%±1.99%,对照组为97.92%±0.49%(P<0.05);噪声组反应幅值为16.00±1.04 dB SPL,对照组为20.27±0.70 dB SPL(P<0.05);噪声组信噪比为12.66±1.55 dB,对照组为20.95±0.89 dB(P<0.01)。SISI 6 kHz与TEOAE 3.5~4.5 kHz频段信噪比负相关(P<0.05)。结论高频率SISI联合提高筛查标准的TEOAE指标可为噪声性听力损失提供早期的诊断途径。 Objective To study the value and significance of short increment sensitivity index(SISI) and transient evoked otoacoustic emissions(TEOAE) in the hearing screening of noise-induced deafness with normal pure tone hearing threshold. Methods Fifty young adults with normal pure tone hearing threshold and a history of noise exposure participated in the study, in addition fifty subjects without noise exposure were included as the control group. The average age of the experimental group and the control group were 19.58±1.03 and 19.90±1.20 years old, respectively. Pure tone audiometry, SISI and TEOAE were performed. Statistical analysis was performed to analyze differences between two groups. Results The pure tone hearing thresholds from 0.25 kHz to 8 kHz in control group and noise group were less than 20 dB HL. No significant differences were found(P>0.05). The SISI of noise group were higher than that of normal group, and significant differences were found(P<0.01). The total correlation rate of TEOAE waveform in noise group and control group was 92.32%±1.99% and 97.92%±0.49%, respectively(P<0.05). The response intensity in noise group and control group was 16.00±1.04 dB SPL and 20.27±0.70 dB SPL(P<0.05). The overall SNR in the noise group and control group was 12.66±1.55 dB and 20.95±0.89 dB, respectively(P<0.01). SISI of 6 kHz was negative correlated with TEOAE 3.5~4.5 kHz SNR(P<0.05). Conclusion High frequency SISI combined with TEOAE of higher screening standards can provide an early diagnosis strategy for noise-induced deafness.
