摘要
目的探讨基于稳态模型(HOMA2)计算的不同胰岛素敏感指数(ISI)下妊娠期糖尿病(GDM)患者的血脂特征。方法本研究为回顾性分析,选取西北妇女儿童医院收治的84例GDM患者和100例糖耐量试验正常者为研究对象,收集其妊娠早、中、晚期总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)等相关资料。使用HOMA2计算ISI,并将GDM患者分为胰岛素敏感缺陷组(n=32)和胰岛素敏感组(n=52),比较三组妊娠妇女的血脂特征。结果三组孕妇妊娠早、中、晚期TG水平的差异均有统计学意义(H值分别为21.580、13.287、10.914,P<0.05),其中胰岛素敏感缺陷组的TG水平在三个时期均高于胰岛素敏感组和正常妊娠组。三组孕妇在妊娠中、晚期的LDL-C水平差异有统计学意义(H值分别为11.700、12.000,P<0.05),其中胰岛素敏感缺陷组的LDL-C水平在妊娠中期低于正常妊娠组,在妊娠晚期低于胰岛素敏感组和正常妊娠组。三组孕妇妊娠早、中、晚期TC水平(H值分别为1.027、5.667、5.323,P>0.05)和HDL-C水平(H值分别为2.957、0.731、3.820,P>0.05)的差异均无统计学意义。结论不同胰岛素抵抗下的GDM患者血脂水平存在显著差异,胰岛素敏感缺陷者的TG较高而LDL较低。
Objective To investigate the lipid characteristics of gestational diabetes mellitus(GDM)patients with different insulin sensitivity index(ISI)calculated based on homeostasis model assessment(HOMA2).Methods Total cholesterol(TC),triglyceride(TG),high-density lipoprotein(HDL-C),low-density lipoprotein(LDL-C)and other related data were collected retrospectively in the first,second and third trimester of pregnancy in 84 patients with GDM and 100 normal subjects with glucose tolerance test in Northwest Women and Children′s Hospital.HOMA2 was used to calculate ISI,and GDM patients were divided into the insulin sensitive deficiency group(n=32)and the insulin sensitive group(n=52).The lipid characteristics of pregnant women in the three groups were compared.Results The TG levels of the three groups in the first,second and third trimester of pregnancy were significantly different(H values were 21.580,13.287 and 10.914,respectively,P<0.05),and the TG levels of the insulin sensitive deficiency group were higher than those of the insulin sensitive group and the normal pregnancy group in the three periods.There were significant differences in LDL-C levels among the three groups in the second and third trimester(H values were 11.700 and 12.000,respectively,P<0.05).The LDL-C level in the insulin sensitive deficiency group were lower than that in the normal pregnancy group in the second trimester and lower than that in the insulin sensitive and normal pregnancy groups in the third trimester.There was no significant difference in TC level(H values were 1.027,5.667 and 5.323,respectively,P>0.05)and HDL-C level(H values were 2.957,0.731 and 3.820,respectively,P>0.05)among the three groups in the first,second and third trimester of pregnancy.Conclusion There are significant differences in blood lipid levels in GDM patients with different insulin resistance.TG is higher and LDL is lower in patients with insulin sensitivity deficiency.
