摘要
目的:探究亚低温辅助干预对心跳呼吸暂停行心肺复苏(Cardiopulmonary resuscitation,CPR)患者的效果。方法:选取我科95例心跳呼吸暂停后行CPR患者,根据随机数字表法分组。对照组47例给予常规干预及心肺复苏,观察组48例在此基础上联合亚低温干预,对比两组患者神经功能预后、肺部功能、免疫指标水平。结果:心肺复苏后第10 d观察组急性生理和慢性健康状况评分Ⅱ(Acute Physiology and Chronic Health EvaluationⅡ,APACHEⅡ)、二氧化碳分压(Partial Pressure of Carbon Dioxide,PCO_(2))、肺静态顺应性、肺动态顺应性、气道峰压、平台压水平低于对照组,格拉斯哥昏迷量表(Glasgow Coma Scale,GCS)评分、脑电双频指数(Bispectral index values,BIS)、氧分压(Partial pressure of oxygen,PO2)高于对照组(P<0.05)。两组患者免疫球蛋白M(Immune globulin M,IgM)、免疫球蛋白A(Immune globulin A,IgA)及免疫球蛋白G(Immune globulin G,IgG)水平无统计学差异。结论:亚低温辅助干预可提高心跳呼吸暂停行CPR患者的神经功能预后效果,降低肺功能损伤。
Objective:To explore the effects of mild hypothermia assisted intervention on Cardiopulmonary resuscitation(CPR)patients.Methods:a total of 95 cardiac apnea patients treated with CPR were divided into two groups according to random number table.47 patients in the control group were received CPR and routine intervention,and 48 patients in the observation group received mild hypothermia intervention on this basis.The neurological prognosis,pulmonary function and immune index levels of the two groups were compared.Results:On the 10th day after CPR,Acute Physiology and Chronic Health Evaluationⅱ(APACHEⅱ)score,Partial Pressure of Carbon Dioxide(PCO_(2)),lung static compliance,lung dynamic compliance,airway peak pressure and platform pressure levels in the observation group were lower than those in the control group(P<0.05).Glasgow Coma Scale(GCS)score,Bispectral index values(BIS)and Partial pressure of oxygen(PO2)in the observation group were higher than those in control group(P<0.05).There was no significant difference in Immune globulin M(IgM),Immune globulin A(IgA)or Immune globulin G(IgG)levels in the two groups.Conclusion:Mild hypothermia adjuvant intervention can effectively improve the prognosis of neurological function,reduce lung function damage in cardiac apnea patients treated with CPR.
