摘要
目的:探讨超声下胎儿髋关节影像学参数与胎儿生长发育指标的相关性。方法:选取于医院行孕晚期产检的88名孕产妇,将存在髋关节发育不良的88例胎儿纳入髋关节发育不良组;另外选取同期于医院行孕晚期产检无异常的88名胎儿纳入健康对照组。两组胎儿均行超声检查获得头围、腹围和股骨长数据资料,经超声检查后以Graf法测量α角、β角及股骨头覆盖率(MR)。比较两组不同孕周(30周、35周及40周)胎儿髋关节影像学参数(α角、β角及MR)、胎儿生长发育指标(头围、腹围及股骨长)。通过Spearman系数分析超声下胎儿髋关节影像学参数与胎儿生长发育指标间的相关性。结果:髋关节发育不良组孕30周、35及40周的α角及MR均显著低于健康对照组,差异具有统计学意义(t_(α角)=15.964,t=23.827,t=21.654;t_(MR)=14.447,t=12.588,t=11.183;P<0.05),β角显著高于健康对照组,差异具有统计学意义(t=19.552,t=16.178,t=12.395;P<0.05)。髋关节发育不良组孕30周、35周及40周头围、腹围及股骨长测量结果均显著大于健康对照组,差异有统计学意义(t_(头围)=3.308,t=1.479,t=2.405;t_(腹围)=3.618,t=6.306,t=7.038;t_(股骨长)=9.141,t=10.281,t=4.757;P<0.05)。经相关性分析,α角及MR与头围、腹围和股骨长呈负相关(r_(α角)=-5.122,r=-4.918,r=-5.028;r_(MR)=-5.108,r=-5.058,r=-5.114;P<0.05),β角与头围、腹围和股骨长呈正相关(r=5.001,r=4.894,r=4.975;P<0.05)。结论:孕晚期髋关节发育不良胎儿的髋关节超声参数及生长发育指标与健康胎儿相比存在明显异常,且各项影像学参数指标间关系密切,在孕晚期产检过程中临床医师应当予以关注。
Objective:To investigate the correlation between the imaging parameters of fetal hip joint under ultrasound and the indicators of fetal growth and development.Methods:The 88 fetuses with hip dysplasia whose mothers underwent antenatal care at the third trimester were conveniently selected and were divided into hip dysplasia group.At the same time,88 fetuses without abnormality whose mothers underwent antenatal care at the third trimester were divided into health control group.Both two groups of fetuses underwent ultrasound examination to obtain head circumference,abdomen circumference and femur length.The Graf method was used to measureαangle,βangle and femoral head coverage ratio(MR)after ultrasound examination.The imaging parameters(αangle,βangle and MR),the indicators of fetal growth and development(head circumference,abdomen circumference and femur length)of hip joint were compared between the two groups at different gestational weeks(30 weeks,35 weeks and 40 weeks).Spearman coefficient was used to analyze the correlation between the imaging parameters of fetal hip joint under ultrasound and the indicators of fetal growth and development.Results:Theαangle and MR of the hip dysplasia group were significantly lower than those of the healthy control group at the 30 weeks,35 weeks and 40 weeks of pregnancy(t_(αangle)=15.964,t=23.827,t=21.654,t_(MR)=14.447,t=12.588,t=11.183,P<0.05),respectively.Theβangle of the hip dysplasia group was significantly higher than that of the healthy control group at different gestational weeks(t=19.552,t=16.178,t=12.395,P<0.05).The measurement results of head circumference,abdomen circumference and femur length of the hip dysplasia group were significantly higher than those of the health control group at 30 weeks,35 weeks and 40 weeks of pregnancy(t_(head circumference)=3.308,t=1.479,t=2.405,t_(abdomen circumference)=3.618,t=6.306,t=7.038,t_(femur length)=9.141,t=10.281,t=4.757,P<0.05),respectively.Correlation analysis confirmed that_(αangle) and MR were negatively correlated
