摘要
目的分析单剂量阿片类药物对阻塞性睡眠呼吸暂停(obstructive sleep apnea,OSA)手术患儿呼吸抑制的影响。方法回顾性选取2019年10月至2021年12月于台州市第一人民医院行扁桃体腺样体切除手术的患儿纳入研究,伴OSA的患儿48例纳入伴OSA组,不伴OSA的患儿48例纳入非OSA组,两组患儿在全身麻醉诱导和建立稳态通气后,采用1μg/kg芬太尼静脉内给药,记录芬太尼给药前后不同时间的潮气量、呼吸频率、呼气末二氧化碳分压,比较两组患儿上述指标变化是否存在差异。结果两组患儿不同时间的潮气量、呼吸频率及呼气末二氧化碳分压比较,组间效应差异均无统计学意义(P>0.05)。两组患儿均未观察到血氧饱和度<92%等任何不良事件,所有患儿均顺利治疗出院。结论在儿科扁桃体腺样体切除手术中,OSA与单剂量芬太尼后中枢呼吸抑制无关。
Objective To analyze the effect of single-dose opioids on respiratory depression in children with obstructive sleep apnea(OSA)surgery.Methods Children who underwent tonsil adenoidectomy in Taizhou First People’s Hospital from October 2019 to December 2021 were retrospectively selected for the study,48 children with OSA were included in OSA group and 48 children without OSA were included in the non-OSA group.After induction of general anesthesia and establishment of steady-state ventilation,the children of two groups were intravenously administered 1μg/kg fentanyl,and the tidal volume,respiratory rate,and end-tidal carbon dioxide partial pressure at different times before and after administration of fentanyl were recorded,to compare whether there are differences in the changes of the above indicators between the two groups.Results There was no significant difference in tidal volume,respiratory rate and end-tidal carbon dioxide partial pressure between the two groups at different time(P>0.05).No adverse events such as blood oxygen saturation<92% were observed in both groups,and all the children were successfully treated and discharged from hospital.Conclusion In pediatric tonsil adenoidectomy,OSA was not associated with central respiratory depression after a single dose of fentanyl.
