摘要
目的应用三维斑点追踪技术(3D-STI)评价尼可地尔对冠状动脉慢血流(CSF)患者左心室功能的治疗作用。方法选取2018年10月至2020年5月于四川省达州市中西医结合医院住院诊断为CSF的患者37例为CSF组,选择同期健康体检者35例为对照组,收集二维超声心动图和三维参数。CSF组患者使用尼可地尔治疗3个月后复查超声心动图。3D-STI技术获得参数包括:左心室整体纵向收缩峰值应变(GLS)、左心室整体圆周收缩峰值应变(GCS)、左心室整体径向收缩峰值应变(GRS)和左心室整体面积收缩峰值应变(GAS)。结果(1)对照组与CSF组尼可地尔治疗前后常规参数左心室舒张末内径(LVEDD)、左心室舒张末容积(LVEDV)、左心室收缩末容积(LVESV)、左心室每搏量(LVSV)、左心室射血分数(LVEF)、二尖瓣舒张早期血流速度(E峰)、二尖瓣舒张晚期血流速度(A峰)、室间隔与左心室侧壁运动速度平均值(Avg e’)、二尖瓣舒张早期血流速度与室间隔和左心室侧壁运动速度平均值比值(E/e’)比较差异无统计学意义(P>0.05);(2)对照组GLS、GRS、GCS、GAS均高于CSF组,治疗后上述参数有所改善,差异均有统计学意义(F=58.125、46.611、59.582、37.434,P<0.05)。结论3D-STI技术可早期发现CSF患者左心室收缩功能损伤,尼可地尔治疗后CSF患者左心室功能有所恢复。
Objective To explore the application value of evaluating left ventricular function in patients with coronary slow flow(CSF)treated with Nicorandil by three-dimensional speckle-tracking imaging(3D-STI).Methods Thirty-seven patients with diagnosed clinically as CSF in Dazhou Municipal Hospital of Integrated Traditional Chinese and Western Medicine from October 2018 to May 2020 were selected as the CSF group.Thirty-five healthy patients underwent physical examination during the same period were selected as control group.Parameters of two-dimensional echocardiography and three-dimensional echocardiography were collected.Echocardiographic parameters of patients in the CSF group was re-examined after three months of Nicorandil treatment.Parameters obtained by 3D-STI technique included global longitudinal strain(GLS),global circumferential strain(GCS),global radial strain(GRS)and global area strain(GAS).Results(1)There was no significant difference in routine parameters left ventricular end-diastolic dimension,left ventricular end diastolic volume,left ventricular end systolic volume,left ventricular stroke volume,left ventricular ejection fraction,E peak,A peak,average velocities e’and E/e’between control group and CSF group before and after Nicorandil treatment(P>0.05).GLS,GRS,GCS and GAS in the control group were higher than those in the CSF group,and the above parameters were improved after treatment,with statistical significance(F=58.125,46.611,59.582,37.434;P<0.05).Conclusion 3D-STI technology application can detect the damage of left ventricular function of CSF patients at an early stage,and the recovery of left ventricular function after Nicorandil treatment.
