摘要
目的了解全球恶性梗阻性黄疸术前胆道引流相关指南的现状并对其进行评价,为恶性梗阻性黄疸患者的治疗提供参考。方法计算机检索PubMed、Embase、中国知网(CNKI)、万方数据库和中国生物医学文献数据库(CBM),收集全球已发布的恶性梗阻性黄疸术前胆道引流相关的临床实践指南(CPGs),检索时限均从建库至2021年6月1日。由2名研究者独立筛选文献、提取资料,4名评价者使用临床指南研究与评价工具Ⅱ(AGREEⅡ)对纳入的指南进行质量评价。结果共纳入18部指南,其中强推荐15部,弱推荐3部,6个领域的平均得分率依次为:范围和目的84.18%,参与人员78.28%,制定的严谨性67.68%,清晰性79.01%,应用性67.94%,编辑独立性79.51%。每部指南包括6个领域,国内指南平均得分率42.10%,而国际指南平均得分率85.52%。结论不同国家和地区恶性梗阻性黄疸术前胆道引流相关指南质量不一,我国相关指南的总体质量与国际指南相比,存在差距,尚需进一步按照国际指南的规范和流程进行完善。各指南推荐的术前胆道引流方式也存在差异。
Objective To investigate and evaluate the current status of clinical guidelines regarding preoperative biliary drainage(PBD)treatment in malignant obstructive jaundice in global,and to provide reference for the treatment of patients with malignant obstructive jaundice.Methods PubMed,Embase,CNKI,Wanfang Database and CBM were electronically searched to collect published clinical practice guidelines(CPGs)related to PBD in malignant obstructive jaundice from inception to Jun.1,2021.Two researchers independently screened the literature and extracted data and four reviewers evaluated the quality of the included guidelines using the Appraisal of Guidelines for Research and EvaluationⅡ(AGREEⅡ).Results A total of 18 guidelines were included,among which 15 were strongly recommended and 3 were weakly recommended.The average scores of the 6 fields were as follows:84.18%for scope and purpose,78.28%for stakeholder involvement,67.68%for rigor of development,79.01%for clarity of presentation,67.94%for applicability,and 79.51%for editorial independence.The average score of the six areas of each guideline was 42.10%for domestic guidelines and 85.52%for international guidelines.Conclusion The quality of guidelines related to PBD for malignant obstructive jaundice varied in different countries and regions.The overall quality of the domestic guidelines was lower than that of the international guidelines,which need to be further improved according to the norms and procedures of the international guidelines.Different guidelines had different recommendations for PBD.
