摘要
目的:比较同期与分期双侧全膝关节置换术治疗膝骨关节炎患者的效果。方法:选取2019年2月至2021年5月该院收治的102例膝骨关节炎患者进行前瞻性研究,按照随机数字表法分为对照组和观察组各51例。对照组行分期双侧全膝关节置换术治疗,观察组行同期双侧全膝关节置换术治疗。比较两组疗效、手术相关指标(手术时间、术中出血量、住院时间)水平、术前和术后6个月膝关节功能[美国纽约特种外科医院(HSS)]评分、炎性指标[血清肿瘤坏死因子-α(TNF-α)、白细胞介素-10(IL-10)和白细胞介素-6(IL-6)]水平,以及并发症发生率。结果:两组治疗总有效率比较,差异无统计学意义(P>0.05);术后6个月,两组HSS评分均高于术前,TNF-α、IL-10和IL-6水平均低于术前,但两组比较,差异无统计学意义(P>0.05);观察组手术时间、住院时间均短于对照组,术中出血量多于对照组,并发症发生率高于对照组,差异有统计学意义(P<0.05)。结论:同期双侧全膝关节置换术治疗膝骨关节炎患者,其缩短手术时间和住院时间的效果优于分期双侧全膝关节置换术,但术中出血量较多,并发症发生率较高。
Objective:To compare effects of simultaneous and staged bilateral total knee arthroplasty for patients with knee osteoarthritis.Methods:A prospective study was conducted on 102 patients with knee osteoarthritis admitted to the hospital from February 2019 to May 2021.They were divided into control group and observation group according to the random number table method,51 cases in each.The control group was treated with staged bilateral total knee arthroplasty,while the observation group was treated with simultaneous bilateral total knee arthroplasty.The efficacy,the operation-related indicator levels(operation time,intraoperative blood loss,hospitalization time),the knee function[hospital for special surgery(HSS)]score before and 6 months after the surgery,the inflammatory indicator levels[serum tumor necrosis factor-α(TNF-α),interleukin-10(IL-10)and interleukin-6(IL-6)],and the incidence of complications were compared between the two groups.Results:There was no significant difference in the total effective rate between the two groups(P>0.05).6 months after the surgery,the HSS scores of the two groups were higher than those before the surgery,and the levels of TNF-α,IL-10 and IL-6 were lower than those before the surgery,but there were no significant differences between the two groups(P>0.05).Further,the operation time and the hospitalization time of the observation group were shorter than those of the control group,the intraoperative blood loss was more than that of the control group,the incidence of complications was higher than that of the control group,and the differences were statistically significant(P<0.05).Conclusions:Simultaneous bilateral total knee arthroplasty in the treatment of the patients with knee osteoarthritis has shorter operation time and hospitalization time than staged bilateral total knee arthroplasty,but the amount of bleeding is more and the incidence of complications is higher.
