摘要
目的:观察水动力清创系统在烧伤患者创面清创中的应用效果。方法:选取2020年7月至2021年7月该院收治的83例严重烧伤患者进行前瞻性研究,以随机数字表法将其分为观察组41例和对照组42例。对照组予以传统清创,观察组予以水动力清创系统清创,比较两组清创效果、二次清创植皮率、创面愈合时间、清创前后创面组织炎性因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]和血清疼痛因子[5-羟色胺(5-HT)、神经肽Y(NPY)]水平。结果:观察组每1%面积清创时间短于对照组,创面细菌培养阳性率低于对照组,7 d植皮存活率高于对照组,差异均有统计学意义(P<0.05);观察组二次清创植皮率低于对照组,创面愈合时间短于对照组,差异均有统计学意义(P<0.05);清创7 d后,两组创面组织IL-6、TNF-α水平均低于清创前,且观察组低于对照组,差异有统计学意义(P<0.05);清创7 d后,观察组血清5-HT、NPY水平均低于对照组,差异有统计学意义(P<0.05)。结论:水动力清创系统应用于烧伤患者创面清创可提高植皮存活率,缩短清创时间和创面愈合时间,降低创面细菌培养阳性率、二次清创植皮率、创面组织炎性因子水平和血清疼痛因子水平,效果优于传统清创。
Objective:To observe application effects of hydrodynamic debridement system in wound debridement of burn patients.Methods:A prospective study was conducted on 83 patients with severe burns admitted to this hospital from July 2020 to July 2021.They were divided into observation group(41 cases)and control group(42 cases)by using the random number table method.The control group was given traditional debridement,while the observation group was given hydrodynamic debridement system debridement.The debridement effect,the skin grafting rate after secondary debridement,the wound healing time,the levels of inflammatory factors[interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)],and the serum pain factor levels[5-hydroxytryptamine(5-HT)and neuropeptide Y(NPY)]in the wound tissue before and after the debridement were compared between the two groups.Results:The debridement time per 1%area in the observation group was shorter than that in the control group,the positive rate of wound bacterial culture was lower than that in the control group,the 7-day skin graft survival rate was higher than that in the control group,and the differences were statistically significant(P<0.05).The skin grafting rate after secondary debridement in the observation group was lower than that in the control group,the wound healing time was shorter than that in the control group,and the differences were statistically significant(P<0.05).After 7 days of debridement,the levels of IL-6 and TNF-αin the wound tissues of the two groups were lower than those before the debridement,those of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).After 7 days of debridement,the levels of serum 5-HT and NPY in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:The hydrodynamic debridement system in wound debridement for the burn patients can improve the survival rate of skin grafting,shorten the time of d
