摘要
目的 探讨三维准连续动脉自旋标记(3D pCASL)在糖尿病相关足部灌注中的应用价值。资料与方法 前瞻性收集2019年3月—2021年2月在中国中医科学院西苑医院就诊的非糖尿病志愿者(A组)和糖尿病患者(病程≤10年为B组,14例;病程>10年为C组,16例)各30例,每例研究对象进行结构像和5次3D pCASL灌注扫描,标记后延迟时间(PLD)分别取1.0、1.5、2.0、2.5、3.0 s。对扫描完的图像进行后处理,融合解剖像,测量并比较3组间足底内侧肌群、中间肌群、外侧肌群及平均值的血流量。结果 PLD=1.0 s时,中间肌群的血流量3组间有统计学差异,C组高于B组(P=0.015);外侧肌群的血流量3组间差异有统计学差异,C组高于A组(P=0.001)。PLD=1.5 s时,内侧肌群、中间肌群、外侧肌群及平均值的血流量3组间差异均有统计学意义,且均为C组高于A组(P=0.018、0.013、0.005、0.002)。结论 当PLD为1.0 s和1.5 s时,3D pCASL对足底血流量的紊乱有敏感性,有望成为糖尿病相关足部灌注研究的新方法。
Purpose To explore the application value of Three-Dimensional pseudo-continuous arterial spin labeling(3D pCASL) in foot perfusion associated with diabetes mellitus. Materials and Methods Thirty non-diabetic patients(group A) and thirty patients with diabetes,including group B(14 patients with a course of disease ≤10 years) and group C(16 patients with a course of disease >10 years), were prospectively collected from March 2019 to February 2021 in Xiyuan Hospital of China Academy of Chinese Medical Sciences. Each subject underwent anatomical images and 3D pCASL scans with five different post label delay time(PLD), and the PLD was 1.0, 1.5, 2.0, 2.5 and 3.0s respectively. The perfusion image was fused with the anatomical image, and the mean blood flow of plantar muscle(the intermediate muscle,the middle muscle, the lateral muscle, the average value of plantar muscle) was measured on the fused image. Results When the PLD was 1.0 s, the blood flow of the middle muscle group was statistically different among the three groups, and the post hoc test showed that the blood flow of group C was significantly higher than that of group B(P=0.015). There was statistical difference in the blood flow of the lateral muscle group among the three groups, and the post hoc test showed that the blood flow of group C was significantly higher than that of group A(P=0.001). When PLD was 1.5 s, there were significant differences in the blood flow of medial muscle group, intermediate muscle, lateral muscle group as well as the average value of plantar muscle group among the three groups, and all blood flows of group C were all significantly higher than that of group A(P=0.018, P=0.013, P=0.005, P=0.002, respectively). Conclusion When PLD are set to 1.0 s and 1.5 s, 3D pCASL are sensitive to the disturbance of plantar blood flow and are expected to become a new method for diabetic foot perfusion research.
