摘要
目的比较全髋关节置换术与半髋关节置换术治疗老年股骨颈骨折的临床效果。方法回顾性分析2018年1月—2021年10月江苏省泗洪分金亭医院收治的83例老年股骨颈骨折患者的临床资料,根据治疗方式分组,其中40例患者接受半髋关节置换术治疗(对照组),43例患者接受全髋关节置换术治疗(观察组)。比较两组手术临床指标、并发症发生率与术后髋关节Harris评分。结果观察组手术时间(405.70±56.50)min长于对照组,术中出血量(102.50±25.70)mL多于对照组,术后完全负重时间(32.10±3.68)d短于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率(2.33%)低于对照组(17.50%),差异有统计学意义(χ^(2)=3.875,P<0.05)。观察组术后1、3、6个月的髋关节Harris评分分别为(35.80±3.46)分、(42.50±4.85)分、(60.32±6.54)分,高于对照组,差异有统计学意义(t=3.710、4.148、6.922,P<0.05)。结论全髋关节置换术治疗老年股骨颈骨折可加快患者髋关节功能恢复,改善疾病预后,但其会造成更为严重的手术创伤,临床上需根据患者的需求来为其选用合适的术式。
Objective To compare the clinical effect of total hip arthroplasty and hemiarthroplasty in the treatment of femoral neck fracture in the elderly.Methods The clinical data of 83 elderly patients with femoral neck fracture admitted to Sihong Fenjinting Hospital from January 2018 to October 2021 were retrospectively analyzed.According to the treatment method,40 patients received hemiarthroplasty(control group)and 43 patients received total hip replacement(observation group).The clinical indicators,complication rate and Harris score of post-operation of hip were compared between the two groups.Results The operation time(405.70±56.50)min in the observation group was longer than that in the control group,and the intraoperative blood loss(102.50±25.70)mL was more than that in the control group,the total weight-bearing time after operation was(32.10±3.68)d,which was shorter than that of the control group,and the difference was statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 2.33%,lower than that in the control group 17.50%,and the difference was statistically significant(χ^(2)=3.875,P<0.05).The Harris score of post-operation of hip in the observation group at 1 month,3 months and 6months after operation were(35.80±3.46)points,(42.50±4.85)points and(60.32±6.54)points,respectively,higher than that in the control group,and the difference was statistically significant(t=3.710,4.148,6.922,P<0.05).Conclusion Total hip arthroplasty for elderly patients with femoral neck fracture can accelerate the recovery of hip function and improve the prognosis of the disease,but it will cause more serious surgical trauma.Clinically,it is necessary to choose the appropriate surgical method according to the needs of patients.
