摘要
目的探究及总结原发性肾淋巴瘤(PRL)的特征性影像学表现,并与病理结果进行对照,以提高对该疾病的认识。方法回顾性分析我院经病理及免疫组织化学证实的16例PRL患者资料,总结影像特征并与组织病理学结果进行对照。结果病灶均为单侧,左侧10例,右侧6例;腹膜后浸润型5例,单发结节型3例,多发结节型3例,肾盂型3例,肾周型1例,弥漫浸润型1例。肿瘤长径均大于5 cm且无钙化、出血;CT/MRI平扫多表现为密度/信号均匀的软组织肿块影;DWI呈明显高信号;增强实性成分轻中度强化,10例强化均匀,11例有同侧肾动脉包绕或穿行,2例病灶较大同时伴有脾动脉包绕;9例伴有肿块周围或腹膜后肿大淋巴结。9例乳酸脱氢酶升高。免疫组化染色显示CD20(+)93.3%,CD79α(+)66.7%,Bcl-2(+)66.7%,Mum1(+)66.7%,(Ki-67≥50%)60.0%。结论PRL的CT、MRI表现具有特征性,联合实验室检查及病理结果,有助于该疾病的鉴别诊断。
Objective To summarize the imaging features and pathological correlation of primary renal lymphoma(PRL).Methods Imaging studies and immunohistochemical data of 16 patients with pathologically confirmed PRL were retrospectively analyzed.Results All lesions were unilateral involving the left(10)or right(6)kidney.The tumor presented as retroperitoneal infiltration(5),solitary(3)or multiple(3)nodules,renal sinus(3),perirenal(1),or diffuse renal infiltration(1).The maximum size of the tumor was greater than 5 cm with necrotic or cystic changes(5)and no calcification and hemorrhage.The main CT/MRI manifestation was soft tissue mass with reduced diffusion,mild to moderate homogeneous contrast enhancement(10).There was ipsilateral renal artery(11)and splenic artery(2)encasement as well as peripheral and retroperitoneal lymphadenopathy(9).Lactate dehydrogenase was elevated in 9 patients.Immunohistochemical analysis showed positive CD20(93.3%),CD79α(66.7%),Bcl-2(66.7%),Mum1(66.7%),and Ki-67≥50%(60.0%).Conclusion CT/MRI features combined with immunohistochemical analysis aid the diagnosis of PRL.
