摘要
目的探讨系统化路径管理在先天性胆总管囊肿患儿围手术期快速康复护理中的应用价值。方法选取苏州大学附属儿童医院普外科2018年12月至2021年1月收治的60例行胆总管囊肿手术的患儿。其中,2018年12月至2019年10月的30例患儿采用传统围手术期护理纳入对照组,2019年11月至2021年1月的30例患儿采用以系统化路径管理为导向的快速康复护理纳入试验组。比较两组患儿总住院天数、术后住院天数、术后疼痛评分、照护者焦虑评分、住院费用、照护者出院满意度。结果试验组患儿的总住院天数、术后住院天数、住院费用及术后第1、2、3、4天的疼痛评分均低于对照组,照护者焦虑评分术前、术后第1天、术后1周、术后1个月均低于对照组,照护者出院满意度高于对照组,差异均有统计学意义(P<0.05)。结论以系统化路径管理为导向的快速康复护理模式应用于胆总管囊肿患儿围手术期,可促进患儿术后恢复,减少陪护者焦虑情绪,适于临床推广应用。
Objective To explore the application value of systematic path management in perioperative rapid rehabilitation nursing of children with congenital choledochal cyst.Methods A total of 60 children who underwent choledochal cyst surgery in the Department of General Surgery,Children’s Hospital of Soochow University from December 2018 to January 2021 were selected.30 children treated in the hospital from December 2018 to October 2019 were included in the control group and received traditional perioperative nursing,while 30 children treated in the hospital from November 2019 to January 2021 were included in the experimental group and received rapid rehabilitation nursing guided by systematic path management.The total hospital stay,postoperative hospital stay,postoperative pain score,caregiver anxiety score,hospital cost,and caregiver satisfaction at discharge were compared between the two groups.Results The total hospital stay,postoperative hospital stay,hospital cost and pain scores on days 1,2,3,and 4 after operation of children in the experimental group were lower than those of the control group;the caregiver anxiety score before operation,on day 1,week 1,and month 1 after operation were lower than those of the control group;the caregiver satisfaction at discharge was higher than that of the control group,with statistically significant differences(P<0.05).Conclusion The rapid rehabilitation nursing mode guided by systematic path management applied in perioperative period of children with choledochal can promote postoperative recovery of children and reduce anxiety of caregivers.Therefore,it is suitable for clinical promotion and application.
