摘要
目的观察嗜酸性粒细胞性鼻窦炎患儿外周血miR-20b检测的临床意义及对临床预后的影响。方法以2019年1月—12月收治的50例嗜酸性粒细胞性鼻窦炎患儿为嗜酸鼻窦炎组。另同期选取50例健康体检儿童为对照组。3组患儿均行外周血miR-20b、嗜酸性粒细胞及免疫功能指标检测,观察3组患儿miR-20b、嗜酸性粒细胞、免疫功能指标的染色表现及表达情况及相关性,观察嗜酸鼻窦炎组患儿治疗前miR-20b水平对治疗6周时临床预后的影响。结果HE染色使miR-20b分子核呈深蓝色,miR-20b分子胞浆及纤维组织表现出深浅不等的红色。对照组、非嗜酸鼻窦炎组、嗜酸鼻窦炎组的miR-20b、嗜酸性粒细胞、CD3^(+)、CD4^(+)表达水平依次提升,而CD8^(+)表达水平依次下降(P<0.05)。嗜酸性粒细胞性鼻窦炎患儿miR-20b水平与嗜酸性粒细胞水平、CD3^(+)、CD4^(+)成正相关(r值分别为0.902、0.878、0.863,P<0.05)。嗜酸性粒细胞性鼻窦炎患儿miR-20b水平与CD8^(+)成负相关(r=-0.817,P<0.05)。嗜酸鼻窦炎组50例患儿经药物治疗6周,临床痊愈12例,临床显效14例,临床有效18,临床无效6例。临床痊愈病例、临床显效病例、临床有效病例、临床无效病例治疗前的miR-20b和嗜酸性粒细胞计数水平依次提升,两两比较差异有统计学意义(P<0.05)。结论miR-20b在嗜酸性粒细胞性鼻窦炎患儿外周血中高度表达,其临床高表达可能参与嗜酸性粒细胞的免疫调控过程,且其高表达可能对临床预后产生影响,值得临床关注。
Objective To observe the clinical significance of miR-20b detection in children with eosinophilic sinusitis and its influence on clinical prognosis.Methods A total of 50 children with eosinophilic sinusitis admitted from January to December of 2019 were selected as eosinophilic sinusitis group.50 cases of non-eosinophilic sinusitis were selected as eosinophilic sinusitis group.Another 50 healthy children in the same period were enrolled in the control group.Peripheral blood miR-20b,eosinophils and immune function indexes were detected in all three groups.The expression and correlation of miR-20b,eosinophils and immune function indexes were observed in the three groups.The influence of miR-20b level before treatment on the clinical prognosis at 6 w of treatment in the eosinophilic sinusitis group was observed.Results HE staining showed that the nucleus of miR-20b was dark blue,and the cytoplasm and fibrous tissue of miR-20b showed different shades of red.The expression levels of miR-20b,eosinophils,CD3^(+),CD4^(+)in healthy control group,non-eosinophilic sinusitis group and eosinophilic sinusitis group increased in turn,while CD8^(+)expression level decreased in turn(P<0.05).miR-20b level was positively correlated with eosinophil level,CD3^(+),CD4^(+)in children with eosinophilic sinusitis(r=0.902,r=0.878,r=0.863,P<0.05).There was a negative correlation between miR-20b and CD8^(+)in children with eosinophilic sinusitis(r=-0.817,P<0.05).In eosinophilic sinusitis group after treatment,12 cases were cured,14 cases were markedly effective,18 cases were clinically effective and 6 cases were clinically ineffective.The levels of miR-20b and the eosinophil counts increased in turn before treatment in clinical cured cases,clinical markedly effective cases,clinically effective cases and clinically ineffective cases,and the differences were statistically significant(P<0.05).Conclusion miR-20b is highly expressed in the nasal secretion of children with eosinophilic sinusitis.Its clinical high expression may participate in the
