摘要
目的探讨7种自身抗体(p53、GAGE7、PGP9.5、CAGE、MAGE A1、SOX2、GBU4-5)、糖类抗原72-4(CA72-4)和胃蛋白酶原(PG)联合检测在胃癌中的诊断价值。方法在112例胃癌患者、118例胃部疾病患者和114例体检者中,采用ELISA方法检测血清中7种自身抗体浓度,化学发光法检测PGⅠ、PGⅡ、PGⅠ/PGⅡ和CA72-4的浓度;绘制ROC曲线分析上述指标联合诊断胃癌的效能。结果胃癌组p53、PGⅠ和PGⅠ/PGⅡ水平均低于胃部疾病组与体检组,CAGE、CA72-4和PGⅡ均高于胃部疾病组与体检组(P<0.05)。7种自身抗体、CA72-4、PGⅠ、PGⅡ和PGⅠ/PGⅡ诊断胃癌的敏感度分别为33.0%、13.4%、65.2%、60.7%和35.7%,ROC曲线下面积(AUC)分别为0.613、0.586、0.581、0.553和0.745;7种自身抗体、CA72-4和PG联合诊断胃癌的敏感度为62.5%,AUC可提升至0.773。结论7种自身抗体联合CA72-4和胃蛋白酶原检测对胃癌诊断具有一定的临床价值。
Objective To explore the diagnostic value of combined detection of seven autoantibodies(p53,GAGE7,PGP9.5,CAGE,MAGE A1,SOX2 and GBU4-5),carbohydrate antigen 72-4(CA72-4)and pepsinogen(PG)in gastric cancer.Methods There were 112 cases in gastric cancer group,118 cases in gastric disease group and 114 cases in physical examination group.The serum levels of seven autoantibodies were detected by ELISA,and the serum levels of PGⅠ,PGⅡ,PGⅠ/PGⅡand CA72-4 were detected by chemiluminescence method in three groups;the diagnostic efficiency of seven autoantibodies combined with CA72-4 and PG was analyzed by ROC curve.Results The levels of p53,PGⅠ,PGⅠ/PGⅡin gastric cancer group were lower than those in gastric disease group and physical examination group,while the levels of CAGE,CA72-4,PGⅡwere higher than those in gastric disease group and physical examination group(P<0.05);the sensitivity of seven autoantibodies,CA72-4,PGⅠ,PGⅡand PGⅠ/PGⅡin the diagnosis of gastric cancer was 33.0%,13.4%,65.2%,60.7%and 35.7%respectively,and the AUC was 0.613,0.586,0.581,0.553 and 0.745 respectively;the sensitivity of all the markers combined diagnosis was 62.5%,and the AUC was increased to 0.773.Conclusion The combined detection of seven autoantibodies,CA72-4 and pepsinogen has a certain clinical value for the diagnosis of gastric cancer.
