摘要
目的探讨肩关节镜下双排缝合桥技术治疗习惯性肩关节脱位合并肩袖损伤对患者肩关节功能的影响。方法选取2020年1月至2021年12月福建医科大学附属泉州第一医院收治的120例习惯性肩关节脱位合并肩袖损伤患者为研究对象,根据手术方法的不同将患者分为对照组与试验组,每组60例。对照组采取以单排为主的缝合桥技术,试验组采用以双排为主的缝合桥技术,治疗结束后,比较两组肩关节功能[采用美国加州大学肩关节评分(UCLA)评估]、疼痛程度[采用视觉模拟评分法(VAS)评估]、肩关节功能恢复效果优良率、肩关节活动度(ROM)、术后并发症发生情况及术后6月内再撕脱发生率。结果治疗前,两组UCLA评分、VAS评分比较,差异无统计学意义(P>0.05);治疗后,两组UCLA评分均高于治疗前,VAS评分均低于治疗前,且试验组UCLA评分高于对照组,VAS评分低于对照组,差异有统计学意义(P<0.05);试验组肩关节功能恢复效果优良率高于对照组,差异有统计学意义(P<0.05);治疗前,两组肩关节外旋、前屈度数比较,差异无统计学意义(P>0.05);治疗后,两组肩关节外旋、前屈度数均大于治疗前,且试验组均大于对照组,差异有统计学意义(P<0.05);试验组术后并发症总发生率低于对照组,差异有统计学意义(P<0.05);试验组术后6月内再撕脱发生率低于对照组,差异有统计学意义(P<0.05)。结论肩关节镜下双排缝合桥技术可有效改善习惯性肩关节脱位合并肩袖损伤患者的肩关节功能,减轻术后疼痛感,扩大关节活动度,并可减少术后并发症,降低术后再撕脱发生风险。
Objective To analyze the excellent and good rate of shoulder joint function in the treatment of habitual shoulder dislocation combined with rotator cuff injury by double-row suture bridge technique under shoulder arthroscopy.Methods A total of 120 patients with habitual shoulder dislocation combined with rotator cuff injury who were hospitalized in the Department of Orthopedics of hospital from January 2020 to December 2021 were selected.According to different surgical methods,the suture bridge technology of 60 patients in the comparison group was mainly double-row,while the suture bridge technology of 60 patients in the study group was single row.The University of California shoulder(UCLA)score,visual analogue scale(VAS)score,excellent and good rate of shoulder joint function,range of motion(ROM),total incidence of complications,and incidence of reavulsion within 6 months were compared between the two groups.Results After treatment,the UCLA score in the study group was higher than that in the control group,the VAS score in the study group was lower than that in the control group,and the excellent and good rate of shoulder joint function in the study group was higher than that in the control group,the differences were statistically significant(P<0.05).After treatment,the external rotation and forward flexion ROM of the study group were higher than those of the comparison group,the differences were statistically significant(P<0.05).The total incidence of complications and the incidence of reavulsion within 6 months in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion The double-row suture bridge technique under shoulder arthroscopy can effectively improve the shoulder joint function of patients with habitual shoulder dislocation combined with rotator cuff injury,reduce pain,expand joint range of motion,reduce complications,reduce the risk of reavulsion,and has a certain safety.
