摘要
目的研究外周血微小RNAs(microRNAs)用于肺炎合并脓毒症诊断及预后评价的价值。方法选择45例合并脓毒症的肺炎患者作为肺炎合并脓毒症组;83例未合并脓毒症的肺炎患者作为肺炎组;60例体检的健康者作为对照组。检测外周血miR-let-7d、miR-15b、miR-16、miR-25、miR-92a、miR-103、miR-107、miR-451的表达量,采用急性生理和慢性健康评分Ⅱ(APACHEⅡ)、白细胞计数(WBC)、中性粒细胞计数(NEU)、超敏C反应蛋白(hs-CRP)、降钙素原(PCT)评价肺炎合并脓毒症患者的病情严重程度,采用28d生存情况评价肺炎合并脓毒症患者的预后。结果与对照组比较,肺炎合并脓毒症组、肺炎组的外周血let-7d、miR-15b、miR-107、miR-451表达水平增加,miR-16、miR-25、miR-92a、miR-103表达水平降低(P<0.05);与肺炎组比较,肺炎合并脓毒症组的外周血miR-107表达水平增加,miR-16、miR-25、miR-103表达水平降低(P<0.05),let-7d、miR-15b、miR-92a、miR-451表达水平无统计学差异(P>0.05);外周血miR-16、miR-25、miR-103、miR-107表达水平对肺炎合并脓毒症具有诊断价值且与患者的APACHEⅡ、WBC、NEU、hs-CRP、PCT具有相关性;与肺炎合并脓毒症组中生存患者比较,死亡患者外周血miR-16、miR-25、miR-92a、miR-103表达水平降低,miR-107表达水平增加(P<0.05),let-7d、miR-15b、miR-451表达水平无统计学差异(P>0.05);外周血miR-16、miR-25、miR-92a、miR-103、miR-107表达水平对肺炎合并脓毒症的具有预测价值。结论外周血多种miRs可用于肺炎合并脓毒症的诊断及预后评价。
Objective To study the value of peripheral blood microRNAs(microRNAs)in the diagnosis and prognosis evaluation of pneumonia complicated with sepsis.Methods 45 pneumonia patients complicated with sepsis were selected,83 pneumonia patients without sepsis were selected as the pneumonia group and 60 healthy subjects as the control group.The expression level of miR-let-7 d,miR-15 b,miR-16,miR-25,miR-92 a,miR-103,miR-107,and miR-451 in peripheral blood were measured.The severity of pneumonia complicated with sepsis was evaluated by acute physiology and chronic health scoreⅡ(APACHEⅡ),white blood cell count(WBC),neutrophil count(NEU),high-sensitivity C-reactive protein(hs-CRP),and procalcitonin(PCT),and the prognosis of pneumonia complicated with sepsis was evaluated by 28-day survival.Results Compared with the control group,the expression levels of let-7 d,miR-15 b,miR-107,and miR-451 were increased,while the expression levels of miR-16,miR-25,miR-92 a,and miR-103 were decreased in peripheral blood of pneumonia complicated with sepsis group and the pneumonia group(P<0.05).Compared with the pneumonia group,the expression level of miR-107 in peripheral blood of pneumonia complicated with sepsis group was increased,while the expression levels of miR-16,miR-25,and miR-103 were decreased(P<0.05),there was no significant difference in the expression levels of let-7 d,miR-15 b,miR-92 a,and miR-451(P>0.05).The expression levels of miR-16,miR-25,miR-103,and miR-107 in peripheral blood had diagnostic values for pneumonia complicated with sepsis and were correlated with APACHEⅡ,WBC,NEU,hs-CRP,and PCT.Compared with the surviving patients in pneumonia complicated with sepsis group,the expression levels of miR-16,miR-25,miR-92a,and miR-103 in the peripheral blood of the dead patients were decreased,and the expression level of miR-107 was increased(P<0.05),there was no significant difference in the expression levels of let-7d,miR-15b,and miR-451(P>0.05).The expression levels of miR-16,miR-25,miR-92a,miR-103,and miR-107 in periph
