摘要
目的研究梗阻型轻症急性胆源性胰腺炎(ABP)以不同手术时机进行同期多镜联合治疗的临床效果。方法回顾性分析2018年3月至2020年3月河南中医药大学第五临床医学院(郑州人民医院)收治的110例拟行同期多镜联合治疗的梗阻型轻症ABP患者为研究对象,依据手术时机不同作为分组标准,其中早期接受手术治疗的纳入研究组,延期治疗的纳入对照组。对比手术时间、血淀粉酶水平、拔除鼻胆管时间、住院时间和并发症发生率情况。结果两组手术所需时间、术后5 d血淀粉酶水平、拔除鼻胆管时间对比,差异均无统计学意义(P>0.05);研究组的住院时间较对照组明显缩短,差异有统计学意义(P<0.05);研究组并发症发生率为1.82%,对照组并发症发生率为12.73%,两组并发症发生率对比,差异有统计学意义(P<0.05)。结论梗阻型轻症ABP于早期开展同期多镜联合治疗有效缩短了手术时间和术后康复时间,而且术后并发症较少。
Objective To study the clinical effect of simultaneous multi-endoscopy combined therapy for obstructive mild acute biliary pancreatitis(ABP)at different surgical timing.Methods A retrospective analysis was performed on 110 patients with mild obstructive ABP who were admitted to The Fifth Clinical Medical College of Henan University of Chinese Medicine(Zhengzhou People’s Hospital)from March 2018 to March 2020 and planned to receive simultaneous multi-endoscopy combined treatment as the research objects.According to the different surgical timing,the patients who received early surgical treatment were included in the study group and those who received delayed treatment were included in the control group,and the operative time,blood amylase level,time to nasal duct removal,hospital stay,and complication rate were compared.Results There were no statistically significant differences in the operation time,blood amylase level 5 days after operation,and removal time of nasal bile duct between the two groups(P>0.05).The total length of hospital stay in the study group was significantly shorter than that in the control group,with significant difference(P<0.05).The incidence of complications in the study group was 1.82%,while that in the control group was 12.73%.The difference between the two groups was statistically significant(P<0.05).Conclusion In the early stage of mild obstructive ABP,simultaneous multi-endoscopy combined treatment effectively shortened the operation time and postoperative rehabilitation time,and there was fewer postoperative complications.
