摘要
目的观察双多普勒-Tei(DD-Tei)指数联合肺部超声评估肺动脉高压(PH)新生儿右心室功能及肺水量的价值。方法对40例PH伴呼吸窘迫(PH伴呼吸窘迫组,n=40)、42例无呼吸窘迫PH(PH无呼吸窘迫组,n=42)及同期30例无心肺疾病、无PH及呼吸窘迫症状(对照组,n=30)新生儿行超声心动图及肺部超声检查,以右心室DD-Tei评估右心室功能,以肺部超声评分(LUS)评估肺水量;同时评估新生儿呼吸治疗方式(0/1/2/3级)。比较3组肺动脉压力、右心室DD-Tei指数及LUS差异;分析新生儿右心室DD-Tei指数、LUS、肺动脉压力及呼吸治疗方式的相关性。结果PH伴呼吸窘迫组40例,呼吸治疗方式为1级5例、2级9例,3级26例;PH无呼吸窘迫组42例中,呼吸治疗方式为0级16例,1级26例;对照组30例均为0级。肺动脉压力、右心室DD-Tei指数及LUS在PH伴呼吸窘迫组均高于PH无呼吸窘迫组及对照组,PH无呼吸窘迫组均高于对照组(P均<0.05)。新生儿右心室DD-Tei指数与肺动脉压力、LUS及呼吸治疗方式均呈正相关(r s=0.93、0.85、0.76,P均<0.01);LUS与肺动脉压力及呼吸治疗方式均呈正相关(r_(s)=0.82、0.77,P均<0.01)。结论DD-Tei指数联合肺部超声可评价PH新生儿右心室功能及定量肺水量。
Objective To explore the value of dual Doppler-Tei(DD-Tei)index combined with lung ultrasound for evaluating right ventricular function and lung fluid volume in neonates with pulmonary hypertension(PH).Methods Echocardiography and lung ultrasonography were performed in 40 neonates with PH and respiratory distress(PH with respiratory distress group,n=40),42 with PH without respiratory distress(PH without respiratory distress group,n=42)and 30 with non-cardiopulmonary related diseases and without PH nor respiratory symptoms(control group,n=30).DD-Tei index of the right ventricular was measured to evaluate right ventricular function,while 1ung ultrasound scoring(LUS)was obtained to observe lung fluid volume.All neonates were evaluated for respiratory treatment modalities(grade 0/1/2/3).The pulmonary artery pressure,right ventricular DD-Tei index and LUS were compared among groups.The correlations of neonatal right ventricular DD-Tei index,LUS,pulmonary artery pressure and respiratory treatment modalities were analyzed.Results In PH with respiratory distress group,the respiratory treatment modality was grade 1 in 5 cases,grade 2 in 9 cases and grade 3 in 26 cases,which in PH without respiratory distress group was grade 0 in 16 cases and grade 1 in 26 cases,while in control group was all grade 0 in 30 cases.The pulmonary artery pressure,right ventricular DD-Tei index and LUS in PH with respiratory distress group were higher than those in PH without respiratory distress group and control group,while in PH without respiratory distress group were higher than those in control group(all P<0.05).The right ventricular DD-Tei index of neonate was positively correlated with pulmonary artery pressure,LUS and respiratory treatment modalities in all neonates(r s=0.93,0.85,0.76,all P<0.01),LUS was also positively correlated with pulmonary artery pressure and respiratory treatment modalities(r_(s)=0.82,0.77,both P<0.01).Conclusion DD-Tei index combined with lung ultrasound could evaluate the right ventricular function and quantify
