摘要
目的探讨氢吗啡酮静脉自控镇痛用于老年患者术后镇痛的效果及对血清疼痛介质的影响。方法选取2018年1月至2020年12月我院收治的择期手术患者80例,依据术后镇痛方案的不同分为两组各40例。对照组采用右美托咪定+托烷司琼静脉自控镇痛,研究组采用盐酸氢吗啡酮+托烷司琼静脉自控镇痛。比较两组的镇痛效果和血清疼痛介质水平。结果干预后,研究组的躁动评分、VAS评分均显著低于对照组,Ramsay评分、BCS评分均显著高于对照组(P<0.05)。干预后,研究组的血清疼痛介质(NO、5-HT和PGE_(2))水平均显著低于对照组(P<0.05)。结论氢吗啡酮静脉自控镇痛应用于老年患者术后镇痛的效果显著,可降低血清疼痛介质水平,具有较高的临床应用价值。
Objective To explore the effect of hydromorphone patient-controlled intravenous analgesia applied in postoperative analgesia of elderly patients and its impact on serum pain mediators.Methods A total of 80 patients with elective surgery admitted to our hospital from January 2018 to December 2020 were selected and divided into two groups according to the different postoperative analgesia regimens,with 40 cases in each group.The control group received dexmedetomidine+tropisetron for patient-controlled intravenous analgesia,while the study group received hydromorphone hydrochloride+tropisetron for patient-controlled intravenous analgesia.The analgesic effects and levels of serum pain mediators were compared between the two groups.Results After intervention,the restlessness score and VAS score of the study group were significantly lower than those of the control group,and the Ramsay score and BCS score were significantly higher than those of the control group(P<0.05).After intervention,the levels of serum pain mediators(NO,5-HT and PGE_(2))of the study group were significantly lower than those of the control group(P<0.05).Conclusions Hydromorphone patient-controlled intravenous analgesia applied in postoperative analgesia of elderly patients has significant effect,which can reduce the levels of serum pain mediators,and has higher clinical application value.
