慢性阻塞性肺疾病急性加重期外周血中NK细胞激活受体和抑制受体表达与肺功能的相关性分析

摘要 目的探讨NK细胞激活受体与抑制受体在慢性阻塞性肺疾病急性加重(AECOPD)患者外周血中的表达水平及与患者肺功能的相关性。方法收集并分离慢性阻塞性肺疾病(COPD)和慢性阻塞性肺疾病急性加重(AECOPD)患者以及健康志愿者外周血单个核细胞,运用荧光定量PCR分析NK细胞激活受体(NKG2D)和抑制受体(NKG2A)表达水平并分析与患者肺功能和动脉血气的相关性。结果NKG2D基因在COPD组和AECOPD组中表达上调,AECOPD组明显高于COPD组,差异有统计学意义(P<0.05);NKG2A基因在COPD组和AECOPD组中表达下调,AECOPD组明显低于COPD组,差异有统计学意义(P<0.05);NKG2D表达水平与患者的FEV_(1)%、FEV_(1)/FVC和PaO_(2)呈负相关,NKG2A表达水平与患者的FEV_(1)%、FEV_(1)/FVC和PaO_(2)呈正相关,差异有统计学意义(P<0.05)。结论AECOPD患者外周血NK细胞激活受体和抑制受体的表达可能与患者病情进展具有相关性。 Objective To investigate the expression levels of NK cell activating receptor and inhibitory receptor in peripheral blood of patients with acute exacerbation chronic obstructive pulmonary disease(AECOPD)and its correlation with pulmonary function.Methods Peripheral blood mononuclear cells from chronic obstructive pulmonary disease(COPD),AECOPD and healthy volunteers were collected and isolated.The expression levels of NK cell activating receptor(NKG2D)and inhibitory receptor(NKG2A)were analyzed by fluorescence quantitative PCR.The correlation with pulmonary function and arterial blood gas was analyzed.Results The expression levels of NKG2D gene were up-regulated in COPD and AECOPD group,and were significantly higher in AECOPD group than that in COPD group.The expression levels of NKG2A gene were down-regulated in COPD group and AECOPD group,and were significantly lower in AECOPD group than that in COPD group(P>0.05).The expression levels of NKG2D were negatively correlated with FEV_(1)%,FEV_(1)/FVC and PaO_(2),and the expression level of NKG2A was positively correlated with FEV_(1)%,FEV_(1)/FVC and PaO_(2)(P<0.05).The expression levels of NKG2A were positively correlated with FEV_(1)%,FEV_(1)/FVC and PaO_(2)(P<0.05).Conclusion The expression of activating and inhibitory receptors of NK cells in peripheral blood of patients with AECOPD may be related to the progression of the disease.
