摘要
详细介绍1例1岁半加菲猫肾脏B型淋巴瘤病例的诊治过程,为同行诊治猫淋巴瘤提供参考。患猫临床表现主要为严重贫血,可视黏膜黄染,轻度脱水,可视粘膜再充盈时间(CRT)>2s,精神食欲差,体重降低,消瘦,多饮多尿,腹部触诊肾脏质地坚实,肿大;实验室检查发现,红细胞(RBC)、血红蛋白(HGB)、红细胞压积(HCT)严重偏低,血液尿素氮(BUN)、球蛋白(GLB)、血磷(PHOS)、胆红素(TBIL)升高;腹部超声提示双肾严重不规则肿大,皮质回声明显升高;肠系膜淋巴结肿大;少量腹腔液性暗区。肾脏穿刺送检,采用分子生物学(PARR-PCR)聚合酶链式反应,诊断结果提示B细胞来源的淋巴瘤。在准备开展COP(淋巴瘤基于环磷酰胺、长春新碱、泼尼松的一种化疗方案)治疗过程中,猫因为严重肾功能不全及低血容量性休克,最终死亡。
The diagnosis and treatment of renal type B lymphoma in a 1.5-year-old Garfield cat were introduced in this paper.The main clinical manifestations of the cats were severe anemia,yellow staining of the Mucosa,mild dehydration,CRT > 2s,poor appetite,weight loss,weight loss,excessive drinking of urine,abdominal palpation of the kidney texture firm,swelling,and laboratory examination showed that RBC,HGB,HCT were severely low,bun,GLB,PHOS,TBIL were elevated,abdominal ultrasound revealed severe irregular enlargement of both kidneys with elevated cortical echogenicity,Mesenteric lymphadenopathy;a small amount of peritoneal fluid Scotoma.A kidney biopsy,by using molecular biology(PARR-PCR)polymerase chain reaction,the diagnosis of b-cell lymphoma was confirmed,and the cat died of severe renal insufficiency and hypovolemic shock in preparation for COP.
