摘要
目的比较经末节指骨隧道抗菌缝线固定与单纯克氏针治疗陈旧性骨性锤状指的临床疗效。方法自2016年2月至2020年12月,我们对45例陈旧性骨性锤状指患者分别采用经末节指骨隧道抗菌缝线固定法(A组23例)和单纯克氏针法(B组22例)治疗,记录患者的围手术期情况及术后患指活动度、并发症等情况,按Crawford功能评分法评估疗效。结果术后随访12~20个月,平均14.7个月,A组术后患指关节的活动度和术后功能的优良率优于B组(P<0.05);总并发症的发生率两组之间差异无统计学意义(P>0.05),但B组出现再发锤状指的发生率高于A组。结论经末节骨隧道抗菌缝线固定治疗陈旧性骨性锤状指,操作简单,固定牢靠,术后患指恢复功能良好,临床效果满意,值得推广。
Objective To compare the clinical efficacy of antibacterial suture fixation through the distal phalanx tunnel and Kirschner wire alone in the treatment of old bony mallet fingers.Methods From February 2016 to December 2020,45 patients with old bony mallet fingers were treated with antibacterial suture fixation through the distal phalanx tunnel(23 cases in group A)and simple Kirschner wire(22 cases in group B),respectively.The perioperative conditions,postoperative finger mobility and complications of the patients were recorded,and the clinical efficacy was evaluated according to Crawford function score.Results The postoperative follow-up ranged from 12 to 20 months,with an average of 14.7 months.The range of motion of the affected finger joint and the excellent and good rate of postoperative function in group A were better than those in group B(P<0.05).There was no significant difference in the incidence of total complications between the two groups(P>0.05),but the incidence of recurrent mallet fingers in group B was higher than that in group A.Conclusion The treatment of old bony mallet fingers with antibacterial suture fixation through distal bone tunnel is simple and reliable.The function of the affected fingers is good after operation,and the clinical effect is satisfactory.It is worth popularizing.
