摘要
目的:分析乳腺导管原位癌(DCIS)患者中,不同病理特征对DCIS微浸润情况的影响因素。方法:选取144例DCIS患者为对象,所有患者经术后病理学检查,分为DCIS组(n=72)和乳腺导管原位癌微浸润(DCIS-Mi)组(n=72),分析DCIS患者的微浸润情况,并通过单因素、多因素分析发生浸润的影响因素。结果:组间资料相比,DCIS-Mi组绝经人群、有人工流产史、可触及肿块发生率较高,组间差异明显(P<0.05);病理学特征相比,中高级组织学分级、HER2表达、ER、PR等指征,组间差异显著(P<0.05);多因素回归分析显示,导致DCIS-Mi的独立危险因素包括绝经情况、中高级组织学分级、肿瘤最大直径和粉刺样坏死(P<0.05)。DCIS-Mi组患者的PLT、PLT/T水平高于DCIS组(P<0.05)。结论:DCIS患者是否发生微浸润,可通过DCIS-Mi的独立危险因素进行评估,包括绝经、组织学分级、最大肿瘤直径和粉刺样坏死。此外,还可结合PLT和PLT/T血流动力学指标,分析是否为DCIS-mi。
Objective:To analyze the influencing factors of different pathological features on DCIS microinfiltration in patients with ductal breast carcinoma in situ(DCIS).Methods:144 DCIS patients were selected.All patients were divided into DCIS(n=72)and ductal carcinoma in situ(DCIS-Mi)(n=72)in DCIS,and analyzed the influencing factors of infiltration by univariate and multivariate analysis.Results:DCIS-Mi,history of abortion,palpable mass(P<0.05),advanced histological classification,HER2 expression,ER,PR(P<0.05);independent risk factors for DCISMi included menopause,intermediate and advanced histological grade,maximum tumor diameter,and acne-like necrosis(P<0.05).The patients in the DCIS-Mi group had higher PLT and PLT/T levels than those in the DCIS group(P<0.05).Conclusion:Whether Microinfiltration occurs in DCIS patients can be assessed by independent risk factors for DCIS-Mi,including menopause,histological grade,maximum tumor diameter,and acne-like necrosis.In addition,the PLT and PLT/T hemodynamic indicators can also be combined to analyze whether the DCIS-mi.
