摘要
目的:比较高频与低频重复经颅磁刺激(rTMS)治疗抑郁症伴失眠患者的效果。方法:选取2021年1月至2022年1月该院收治的80例抑郁症伴失眠患者进行前瞻性研究,按随机数字表法分为高频组、低频组各40例。两组均予以常规药物治疗,在此基础上,高频组给予10 Hz rTMS治疗,低频组给予1 Hz rTMS治疗,两组均持续治疗4周。比较两组治疗前后抑郁程度[汉密尔顿抑郁量表(HAMD)]、睡眠质量[阿森斯失眠评定量表(AIS)]、脑电图指标[听觉事件相关电位P300与失匹配负波(MMN)的潜伏期、波幅]水平,以及不良反应发生率。结果:治疗后,两组HAMD、AIS评分均低于治疗前,差异有统计学意义(P<0.05),但组间比较差异无统计学意义(P>0.05);治疗后,两组P300和MMN的潜伏期均短于治疗前,P300、MMN的波幅均大于治疗前,且高频组的P300、MMN潜伏期短于低频组,波幅大于低频组,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:高频和低频rTMS均能有效改善抑郁症患者的抑郁情绪及失眠症状,但高频rTMS更有利于改善认知功能,临床可根据患者认知功能障碍情况,灵活选择最佳治疗方式。
Objective:To compare effects of high frequency and low frequency repetitive transcranial magnetic stimulation(rTMS)in treatment of depressive patients with insomnia.Methods:80 patients with depression and insomnia admitted to this hospital from January 2021 to January 2022 were selected for the prospective study.They were divided into high frequency group and low frequency group accordi ng to random number table method,40 cases in each group.Both groups were treated with conventional drugs.On this basis,the high frequency group was treated with 10 Hz rTMS,while the low frequency group was treated with 1 Hz rTMS.Both groups were treated for 4 weeks.The degree of depression[Hamilton depression scale(HAMD)],the sleep quality[Asens insomnia scale(AIS)],the EEG indicator levels[latencies of auditory event-related potential P300 and mismatch negativity(MMN),amplitude],and adverse reactions were compared between the two groups before and after the treatment.Results:After the treatment,the HAMD and AIS scores of the two groups were lower than those before the treatment,and the differences were statistically significant(P<0.05);however,there were no statistical differences between the two groups(P>0.05).After the treatment,the latencies of P300 and MMN in the two g roups was shorter than those before the treatment,and the ampl itudes of P300 and MMN were greater than those before the treatment;the latencies of P300 and MMN in the high frequency group were shorter than those in the low frequency group,and the amplitudes were greater than those in the low frequency group;and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Both high-frequency and low-frequency rTMS can effectively improve the depression and insomnia symptoms in the patients with depression,but high-frequency rTMS is more conducive to improving the cognitive function.Clinically,the best treatment can be flexibly selected according to the
