摘要
目的观察热敏点三伏三九贴敷治疗寒湿痹阻型膝骨性关节炎(KOA)的效果及其对T淋巴细胞的影响。方法将2018年6月—2019年7月在江西中医药大学附属医院风湿病科接受三伏三九贴敷治疗的120例寒湿痹阻型KOA患者,随机分为治疗组和对照组,每组60例。分别在热敏点、传统穴位处,于三伏和三九的第1天,贴敷自制穴位贴,6次,共1年。于贴敷前后分别进行中医证候量表及骨性关节炎指数可视化量表(WOMAC)评分;测定肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)的表达水平及T淋巴细胞亚群计数。结果治疗后,与对照组比较,治疗组中医证候积分改善的总有效率更高(P<0.05);治疗组的WOMAC评分下降更明显(P<0.05);治疗组的IL-1β表达水平明显下降(P<0.05),TNF-α及IL-6则差异无统计学意义;2组T淋巴细胞计数差异无统计学意义(P>0.05)。结论热敏点三伏三九贴敷疗法可改善寒湿痹阻型KOA症状,降低IL-1β水平,但对T淋巴细胞影响有限。
Objective To explore the therapeutic effect of dog days application and sanjiu application on heat-sensitive acupoints therapy in the treatment of knee osteoarthritis(KOA)of cold-dampness obstruction type and its influence on T lymphocytes.Methods A total of 120 patients with KOA with cold-dampness obstruction type from July 2018 to July 2019 were enrolled and randomly divided into the treatment group and the control group which were treated with dog days application and sanjiu application on heat-sensitive acupoints or traditional acupoints for one year,respectively.The traditional Chinese medicine(TCM)syndrome and the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)score before and after treatment were observed.Moreover,the serum level of cytokines,including tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and interleukin-6(IL-6),and the T lymphocyte counts,such as CD3+,CD4+and CD8+were tested.Results After treatment,compared with the control group,the total effective rate of improved TCM syndrome score in the treatment group was higher(P<0.05).The decrease of WOMAC score was more obvious in the treatment group(P<0.05).The expression level of IL1βin treatment group was significantly decreased(P<0.05),but there was no significant difference between TNF-αand IL-6.There was no significant difference in T lymphocyte count between the two groups(P>0.05).Conclusion Dog days application and sanjiu application on heat-sensitive acupoints therapy can effectively improve the KOA syndrome,relive symptoms,reduce the level of IL1β,but shared limited effect on T lymphocyte counts.
