摘要
目的:分析脑卒中患者发生吞咽障碍的影响因素。方法:依据是否发生吞咽障碍,于我院治疗的189例脑卒中患者被分为吞咽障碍组(67例)与无吞咽障碍组(122例)。比较两组一般临床资料、美国国立卫生研究院卒中评分量表(NIHSS)评分及改良Barthel指数(mBI),分析脑卒中患者发生吞咽障碍的影响因素。结果:189脑卒中患者中吞咽障碍67例,发生率为35.45%。与无吞咽障碍组比较,吞咽障碍组年龄、肌力分级≤3级比例、NIHSS评分、mBI<41分比例均显著升高(P<0.05或<0.01)。多因素Logistic回归分析显示,年龄、NIHSS评分为脑卒中患者发生吞咽障碍的独立危险因素(OR=1.088、1.356,P=0.001、0.049),肌力分级>3级为其独立保护因素(OR=0.265,P=0.003)。结论:年龄及NIHSS评分为影响脑卒中患者吞咽障碍发生的独立危险因素,临床上应当早期对危险因素进行干预。
Objective:To analyze influencing factors of dysphagia in stroke patients.Methods:According to dysphagia or not,a total of 189 stroke patients treated in our hospital were divided into dysphagia group(n=67)and no dysphagia group(n=122).General clinical data,United States national institutes of health stroke scale(NIHSS)score and modified Barthel index(mBI)were compared between two groups.Influencing factors of dysphagia in stroke patients were analyzed.Results:Incidence rate of dysphagia was 35.45%(67/189)in the 189 stroke patients.Compared with no dysphagia group,there were significant rise in age,percentage of muscle strength grade≤3,NIHSS score and percentage of mBI<41 scores in dysphagia group(P<0.05 or<0.01).Multivariate Logistic regression analysis indicated that age and NIHSS score were independent risk factors for dysphagia in stroke patients(OR=1.088,1.356,P=0.001,0.049),and muscle strength grade>3 was its independent protective factor(OR=0.265,P=0.003).Conclusion:Age and NIHSS score are independent risk factors for dysphagia in stroke patients.Intervention for risk factors should be performed early in clinic.
