MCP―1、CEPPIS评分和应激性高血糖水平在重症监护病房VAP患者中的检测及相关危险因素 被引量:1

摘要 目的:探讨单核细胞趋化蛋白1(MCP-1)、改良临床肺部感染评分(CEPPIS)和应激性高血糖水平在重症监护病房呼吸机相关性肺炎(VAP)患者中的检测及相关危险因素。方法:选择341例机械通气患者,分成VAP组(n=139)和非VAP组(n=202)。比较两组MCP-1、CEPPIS评分和应激性高血糖水平,单因素和多因素Logistic回归分析确定VAP发生的独立危险因素。结果:VAP组血清MCP-1和CEPPIS评分明显高于非VAP组(P<0.05)。ROC曲线表示,MCP-1,CEPPIS评分和应激性高血糖对VAP的发生均有预测价值。Logistic回归分析显示,机械通气时间(>7d)、应激性高血糖、抗菌药物使用时间(≥10d)等均是影响发生VAP的独立影响因素。结论:MCP-1、CEPPIS评分和应激性高血糖均可预测VAP的发生,根据诸因素制定干预方案,以降低VAP发生率。 341 mechanical ventilation patients were divided into VAP group(n=139)and non-VAP group(n=202).MCP-1,CEPPIS score and stress hyperglycemia level were compared between the two groups.Univariate and multivariate Logistic regression analysis was used to determine the independent risk factors for VAP.Serum MCP-1 and CEPPIS scores in VAP group were significantly higher than those in non-VAP group(P<0.05).ROC curve indicated that McP-1,CEPPIS score and stress hyperglycemia had predictive value for the occurrence of VAP.Logistic regression analysis showed that mechanical ventilation duration(>7d),stressinduced hyperglycemia and duration of antibiotics use(≥10d)were all independent factors influencing the occurrence of VAP.It can be concluded that MCP-1,CEPPIS score and stress hyperglycemia can all predict the occurrence of VAP,and intervention plans should be formulated according to these factors to reduce the i ncidence of VAP.
