摘要
目的了解2019—2021年鹰潭市人民医院门急诊抗菌药物使用情况,分析不合理处方存在的问题,并提出改进方案。方法随机抽取2019—2021年该院门急诊抗菌药物处方10354张,调查抗菌药物处方不合理使用情况,并对抗菌药物不合理处方存在的问题进行点评分析。结果2019—2021年,该院门急诊抗菌药物处方使用不合理比例分别为3.30%、2.39%和1.71%,且呈逐年下降趋势(P<0.05);247张不合理处方中,抗菌药物使用情况主要为非限制使用级、单一使用、用药目的主要为治疗、给药方式主要为口服;抗菌药物处方不合理率最高的为用药不适宜,其中用法和用量不适宜所占比例最高,且抗菌药物处方用药不适宜所占比例呈逐年上升趋势(P<0.05),处方书写不规范所占比例呈逐年下降趋势(P<0.05)。结论2019—2021年该院门急诊抗菌药物的使用总体符合临床规定,抗菌药物使用不合理处方比例整体呈下降趋势,但仍存在不合理现象,对门急诊抗菌药物处方进行点评分析,有助于促进医院合理使用抗菌药物,继而改善抗菌药物不合理使用现状,并为门急诊不合理用药提供解决方案,从而保障患者用药安全。
Objective To understand the use of antimicrobial agents in outpatient and emergency departments in Yingtan People′s Hospital from 2019 to 2021,analyze the problems of irrational prescriptions,and put forward improvement plans.Methods Randomly selected 10354 outpatient and emergency antimicrobial agents of the hospital from 2019 to 2021,investigated the irrational use of antibiotic prescriptions,and reviewed and analyzed the problems existing in irrational antibiotic prescriptions.Results From 2019 to 2021,the proportion of irrational use of antimicrobial agents in outpatient and emergency departments in the hospital was 3.30%,2.39%,and 1.71%,respectively.From 2019 to 2021,the proportion of irrational prescriptions of antimicrobial agents in outpatient and emergency departments in the hospital showed a gradual downward trend(P<0.05);among the 247 irrational prescriptions,the use of antimicrobial agents is mainly unrestricted use level,single use,the main purpose of medication is treatment,and the method of administration is mainly oral;from 2019 to 2021,the prescription of antimicrobial agents in outpatient and emergency departments of the hospital are unreasonable,the highest rate was inappropriate medication,of which the proportion of inappropriate usage and dosage was the highest,and the proportion of inappropriate antibiotic prescriptions in outpatient and emergency departments in the hospital from 2019 to 2021 was on the rise(P<0.05),and the proportion of prescription writing showed a downward trend(P<0.05).Conclusion From 2019 to 2021,the use of antimicrobial agents in the outpatient and emergency departments of the hospital generally conform to clinical regulations,and the proportion of irrational prescriptions for antimicrobial agents is generally decreasing,but there are still irrational phenomena.Comments and analysis on the prescriptions of antimicrobial agents in outpatient and emergency departments are helpful to promote the rational use of antimicrobial agents in hospitals will improve the current
