摘要
目的探讨T2DM患者内脏脂肪面积(VFA)与糖尿病周围神经病变(DPN)的相关性。方法选取2020年10月至2021年9月于我院内分泌科住院的T2DM患者283例,采用生物电阻抗法测定VFA,分为VFA>100 cm^(2)组[VA(+),n=140]及VFA≤100 cm^(2)组[VA(-),n=143],记录人口学资料、体格检查及实验室指标,检测下肢震动感觉阈值(VT)、腓肠神经振幅(SNA)及传导速度(SNCV)等,比较两组差异,分析DPN的影响因素。结果与VA(-)组比较,VA(+)组高血压、DR患病率、神经检查异常检出率及VT升高(P<0.05),SNA、SNCV降低(P<0.05)。多元回归分析显示,VFA是SNA和SNCV的影响因素。结论内脏脂肪是T2DM患者DPN的危险因素。
Objective To investigate the relationship between visceral fat area and diabetic peripheral neuropathy(DPN)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 283 T2DM hospitalized patients in the Department of Endocrinology in our hospital were selectedin this study from October 2020 to September 2021.VFA was measured by bioelectrical impedance method and divided into VFA>100 cm^(2) group[VA(+),n=140]and VFA≤100 cm^(2) group[VA(-),n=143].Demographic data,physical examination and laboratory indicators were recorded.The threshold of constant vibration sensation,sural nerve amplitude and conduction velocity were measured.The differences between the two groups were compared and the influencing factors for DPN were analyzed.Results Compared with VA(-)group,the prevalence of hypertension,DR,abnormal detection rate of nerve examination and vibration threshold increased(P<0.05),and the amplitude and conduction velocity of sural nerve decreased in VA(+)group(P<0.05).Multiple regression analysis showed that VFA was the influencing factor for sural nerve amplitude and conduction velocity.Conclusion Visceral fat was an important risk factor for DPN in patients with T2DM.
