摘要
目的 观察二甲双胍联合格列美脲对2型糖尿病(T2DM)患者控糖效果的观察。方法 选取我院2021年6月~2022年3月期间收治的T2DM患者96例,通过系统随机化法分别纳入单药组与联合组,各48例。单药组仅予二甲双胍治疗,联合组在单药组方案上联合格列美脲进行治疗,两组均按各自方案持续治疗3个月。比较两组治疗前后血糖指标[空腹血糖(FBG)、餐后2h血糖(2h-FBG)、糖化血红蛋白(HbA1c)]、胰岛素分泌指数(HOMA-β)、胰岛素抵抗指数(HOMA-IR)、ISI以及血清连接肽(C-P)、ADPN水平差异,并对不良反应情况进行分析比较。结果治疗后,两组FBG、2h-FBG、HbA1c、HOMA-IR水平较治疗前均显著下降,且联合组显著低于单药组(P <0.05);两组HOMA-β、ISI、ADPN水平较治疗前均显著上升,且联合组显著高于单药组(P <0.05);两组患者C-P水平以及不良反应情况无显著差异(P> 0.05)。结论 二甲双胍联合格列美脲应用于T2DM患者时,在提升控糖效果、胰岛素敏感指数以及ADPN水平上均可显著获益,可改善患者胰岛功能,临床效果显著。
Objective To observe the glucose control effects of metformin combined with glimepiride on patients with type 2 diabetes mellitus(T2 DM).Methods 96 patients with T2 DM who were treated in the hospital from June 2021 to March 2022 were selected and divided into single-drug group and combination group by systematic randomization method,with 48 cases in each group.The single-drug group was only treated with metformin while the combination group was combined with glimepiride based on the regimen in the single-drug group.Both groups were treated continuously for 3 months according to their respective regimen.The blood glucose indicators[fasting blood glucose(FBG),2 h postprandial blood glucose(2 h-FBG),glycated hemoglobin(HbA1 c)],homeostasis model assessment of insulin secretion index(HOMA-β),homeostasis model assessment of insulin resistance(HOMA-IR),insulin sensitivity index(ISI)and levels of serum connecting peptide(C-P)and adiponectin(ADPN)were compared between the two groups before and after treatment,and the adverse reactions were analyzed and compared.Results After treatment,the levels of FBG,2 h-FBG,HbAlc and HOMA-IR in the two groups were significantly decreased compared with those before treatment,and the levels in combination group were significantly lower than those in single-drug group(P<0.05).The HOMA-β,ISI and ADPN were significantly increased in both groups compared to before treatment,and the above three indicators were significantly higher in combination group than those in single-drug group(P<0.05).There were no significant differences in C-P level and adverse reactions between the two groups(P>0.05).Conclusion Metformin combined with glimepiride for patients with T2 DM can significantly achieve the benefits in improving glucose control effects,insulin sensitivity index and ADPN level,and it can improve the islet function of patients and has significant clinical effects.
