摘要
目的:探讨在触诊阴性乳腺结节切除活检中应用超声引导下导丝定位的效果。方法:按1∶1的比例随机将我院2019年5月至2020年10月收治的62例触诊阴性乳腺结节切除活检患者分为对照组和观察组,各31例,对照组予以常规体表定位,观察组予以超声引导下导丝定位,比较两组的术后病理结果、手术相关指标,术后并发症及复发情况。结果:入组病例中BI-RADS 3类均为良性;BI-RADS 4a类中恶性占2.22%(1/45);BI-RADS 4b类中恶性占55.56%(5/9);BI-RADS 4c类均为恶性,两组病理结果无统计学差异(P>0.05)。观察组手术时间、切除组织量及住院时间分别为(27.82±3.51)min,(42.51±12.20)g,(1.65±0.70)天,均优于对照组(P<0.05)。观察组术后并发症发生率为6.45%,低于对照组(P<0.05)。随访1年,两组复发率无统计学差异(P>0.05)。结论:触诊阴性乳腺结节切除活检患者应用超声引导下导丝定位可以缩短手术和住院时间,减少患者正常乳腺组织损伤,降低术后并发症的发生,具有临床应用价值。
Objective:To investigate the role of ultrasound-guided guidewire localization in the resection and biopsy of negative breast nodules.Methods:A total of 62 patients with single breast nodules with negative palpation admitted to our hospital from May 2019 to October 2020 were selected and randomly divided into the observation group and the control group with a ratio of 1∶1,with 31 cases in each group.The control group was given conventional body surface positioning,while the observation group was given ultrasound-guided guidewire positioning.The operation time,resected tissue volume,postoperative pathological results,length of hospital stay,postoperative complications and recurrence were compared between the two groups.Results:The BI-RADS 3 categories were benign in the enrolled cases.The malignant rate of BI-RADS 4a was 2.22%(1/45).Malignancy accounted for 55.56%(5/9)in BI-RADS 4b;Bi-rads 4c were all malignant,and there was no significant difference in pathological results between the two groups(P>0.05).The operation time,tissue volume and hospital stay of the observation group were(27.82±3.51)min,(42.51±12.20)g,(1.65±0.70)days,respectively,which were better than those of the control group(P<0.05).The postoperative complication rate of the observation group was 6.45%,which was lower than that of the control group(P<0.05).There was no significant difference in the recurrence rate between the two groups(P>0.05).Conclusion:Ultrasound-guided guidewire positioning can shorten the operation and hospital stay of patients with negative palpated breast nodules resection and biopsy,reduce the damage of normal breast tissue,and reduce the occurrence of postoperative complications,which has clinical application value.
