摘要
目的从“治未病”角度出发调查肺癌患者预定化疗前的症状负担和症状群。方法选择中国中医科学院广安门医院等5家医院肺癌患者,使用安德森症状量表中医版(MDASI-TCM)在预定化疗前的24 h内进行症状评分,通过因子分析研究症状群。结果共纳入患者168例,肺癌患者预定化疗前常见的症状依次是疼痛(90.5%)、咳嗽(76.8%)、疲乏(75.6%)、气短(69.0%)、失眠(66.1%)、口干(65.5%)、咳痰(62.5%)。常见的症状干扰是一般活动(56.0%)和情绪(56.0%)。提取到3个症状群:心理消化症状群(悲伤、烦躁、苦恼、嗜睡、胃口差、恶心、心慌)、病感症状群(口苦、健忘、口干、失眠)和肺癌特异症状群(咳嗽、咳痰)。其中心理消化症状群与症状、症状干扰得分的相关系数均大于0.7。结论肺癌患者在预定化疗前便承受多重症状负担,其中心理消化症状群对患者症状及症状干扰影响较大,提示预定化疗前中医治法应重视疏肝和胃。
Objective To investigate the symptom burden and symptom clusters of expected chemotherapy lung cancer patients based on the theory of“prevention before disease”.Methods Lung cancer patients from 5 hospitals including Guang'anmen Hospital of China Academy of Chinese Medical Sciences were chosen.MD Anderson Symptom Inventory-Traditional Chinese Medicine(MDASI-TCM)was used to investigate symptom score within 24 hours before expected chemotherapy.Symptom clusters were researched by factor analysis.Results Totally 168 patients were included.The common symptoms of expected chemotherapy lung cancer patients were pain(90.5%),cough(76.8%),fatigue(75.6%),shortness of breath(69.0%),insomnia(66.1%),dry mouth(65.5%)and expectoration(62.5%).The common symptom interference was general activity(56.0%)and emotion(56.0%).Three symptom clusters were extracted:psychodigestive symptom cluster(sadness,irritability,distress,lethargy,poor appetite,nausea,palpitation),disease symptom cluster(bitter mouth,forgetfulness,dry mouth,insomnia),lung cancer-specific symptom cluster(cough,expectoration).The correlation coefficients between psychodigestive symptom clusters and symptoms,symptom interference scores were all more than 0.7.Conclusion Lung cancer patients bear multiple symptom burden before expected chemotherapy.Among them,the psychodigestive symptom cluster has a greater impact on the symptoms and symptom interference.It is suggested that more attention should be paid to soothing liver and regulating stomach before expected chemotherapy.
