不同飞秒激光制瓣厚度辅助LASIK矫正治疗近视的效果及1年随访观察 被引量:1

Effects of laser-assisted in-situ keratomileusis assisted by different flap thickness created with femtosecond laser in the treatment of myopia and an 1-year follow-up observation

摘要 目的比较不同飞秒激光制瓣厚度辅助制瓣准分子激光原位磨镶术(LASIK)矫正治疗近视的临床效果及其1年随访情况。方法回顾性分析2018年3月至2020年3月在延安大学附属医院眼科接受治疗的180例(360眼)近视患者的临床资料,所有患者均给予飞秒激光辅助LASIK矫正治疗,根据飞秒激光制瓣厚度不同将患者分为90μm角膜瓣组(84例,168眼)和110μm角膜瓣组(96例,192眼),采用眼前节光学相干断层(OCT)检查,比较不同时间段的视力矫正效果(最佳矫正视力、裸眼视力、角膜后表面顶点厚度、屈光度),并随访1年观察视觉质量情况。结果两组患者术前最佳矫正视力、裸眼视力、角膜后表面顶点厚度、屈光度比较差异均无统计学意义(P>0.05);术后1个月、3个月两组患者的各项视力矫正指标均升高,且在术后3个月,90μm角膜瓣组患者的最佳矫正视力、裸眼视力及角膜后表面顶点厚度分别为1.28±0.14、1.18±0.17、(552.31±26.08)μm,明显高于110μm角膜瓣组的1.20±0.15、1.13±0.12、(545.19±20.84)μm,差异均有统计学意义(P<0.05),但两组患者的屈光度比较差异无统计学意义(P>0.05);随访1年发现,90μm角膜瓣组患者的视觉质量得分为(15.23±2.85)分,明显高于110μm角膜瓣组的(13.05±2.34)分,差异有统计学意义(P>0.05)。结论飞秒激光90μm角膜瓣与110μm角膜瓣分别辅助LASIK矫正治疗在近视患者中均能起到较好的临床效果,但90μm角膜瓣更能有效改善视力及散光程度,可较快恢复患者视力,且预后视觉质量更好。 Objective To compare the clinical effects of laser-assisted in-situ keratomileusis(LASIK)assisted by different flap thickness created with femtosecond laser in the treatment of myopia,and analyze the 1-year follow-up results.Methods The clinical data of 180 myopic patients(360 eyes)treated with LASIK in the Department of Ophthalmology,Affiliated Hospital of Yan'an University from March 2018 to March 2020 were retrospectively analyzed.The patients were divided into 90μm corneal flap group(84 patients,168 eyes)and 110μm corneal flap group(96 patients,192 eyes)according to the flap thickness created with femtosecond laser.The visual acuity correction effects(best corrected visual acuity,uncorrected visual acuity,posterior corneal vertex thickness,and diopter)in different time periods were comparatively analyzed.All patients were followed up for 1 year to observe the visual quality.Results There was no statistically significant difference between the two groups before operation in terms of the best corrected visual acuity,uncorrected visual acuity,posterior corneal vertex thickness,and diopter(P>0.05).These indicators in the two groups increased at 1 month and 3 months after operation.At 3 months after operation,the best corrected visual acuity,uncorrected visual acuity,and posterior corneal vertex thickness of the 90μm corneal flap group were 1.28±0.14,1.18±0.17,and(552.31±26.08)μm,which were significantly higher/larger than 1.20±0.15,1.13±0.12,and(545.19±20.84)μm of the 110μm corneal flap group(P<0.05),but there was no difference in diopter between the two groups(P>0.05).The 1-year follow-up results found that the visual quality score of the 90μm corneal flap group was higher than that of the 110μm corneal flap group:(15.23±2.85)vs(13.05±2.34),P>0.05.Conclusion LASIK assisted by 90μm of corneal flap or 110μm of corneal flap created with femtosecond laser is effective in the treatment of myopic patients.However,LASIK assisted by 90μm of corneal flap can improve visual acuity and astigmatism more e
