摘要
目的 观察滋肾促卵汤联合氯米芬对卵巢早衰患者中医症候及卵巢血流的作用。方法 将我院60例卵巢早衰患者随机分为治疗组与对照组各30例。两组在激素替代治疗基础上,对照组服用氯米芬,治疗组服用滋肾促卵汤+氯米芬。治疗后,比较两组临床疗效、中医症候积分、雌二醇(E)、卵泡刺激素(FSH)、黄体生成素(LH)表达情况及卵巢间质动脉峰值流速(PSV)、阻力指数(RI)、搏动指数(PI)值的变化。结果 治疗后,治疗组愈显率、总有效率均显著高于对照组(P<0.05);治疗组中医症候闭经、四肢倦怠、腰骶酸痛、头晕耳鸣、性欲减退、不思饮食、食后腹胀、便溏积分显著低于对照组(P<0.05);治疗组E2表达、PSV值显著高于对照组,FSH、LH表达、RI、PI值显著低于对照组(P<0.05)。结论 采用滋肾促卵汤联合氯米芬能提高对POF患者的治疗疗效,有效改善中医症状,作用机制与性激素水平变化和卵巢血流动力学改善有关。
Objective To observe the effect of Zishen Culuan Decoction combined with clomiphene on TCM symptoms and ovarian blood flow in patients with premature ovarian failure(POF). Methods Sixty patients with POF were randomly divided into treatment group and control group with 30 cases in each group. On the basis of hormone replacement therapy,control group was given clomiphene,and treatment group was given Zishen culuan Decoction plus clomiphene. After treatment,the clinical efficacy,TCM symptom score,estradiol(E),follicle-stimulating hormone(FSH),luteinizing hormone(LH)expressions,peak ovarian interstitial arterial velocity(PSV),resistance index(RI),pulsatility index(PI)changes were compared between the two groups. Results After treatment,the cure rate and total effective rate in treatment group were significantly higher than those in control group( P<0.05);the TCM symptoms amenorrhea,limb fatigue,lumbosacral pain,dizziness and tinnitus,hypaphrodisia,auepithyrnia,abdominal distension after eating,stool scores in treatment group were significantly lower than in control group( P<0.05);the expression of Eand PSV in treatment group were significantly higher than in control group,and the expression of FSH,LH,RI,and PI in treatment group were significantly lower than those in control group( P<0.05).Conclusion Zishen culuan Decoction combined with clomiphene can enhance the therapeutic effect of POF patients,andimprove the symptoms of TCM effectively. The mechanism is related to the changes of sex hormone levels and the improvement of ovarian hemodynamics.
