摘要
目的:探究血液灌流(HP)联合盐酸戊乙奎醚对急诊重度有机磷农药中毒(AOPP)患者氧化应激及微炎症指标的影响。方法:选取2020年2月至2021年12月于石狮市总医院急诊科采用HP治疗的70例AOPP患者,采用随机数字表法分为两组,各35例。对照组采用HP治疗,观察组采用HP联合盐酸戊乙奎醚治疗。比较两组恢复情况,氧化应激[乳酸脱氢酶(LDH)、超氧化物歧化酶(SOD)]及微炎症指标[白细胞介素-6(IL-6)、肿瘤坏死因子α(TNF-α)]。结果:观察组昏迷时间、胆碱酯酶恢复正常时间短于对照组(P<0.05)。治疗1周,两组LDH水平低于治疗前,观察组低于对照组,两组SOD水平高于治疗前,观察组高于对照组(P<0.05)。治疗1周,两组IL-6、TNF-α低于治疗前,观察组低于对照组(P<0.05)。结论:HP联合盐酸戊乙奎醚可改善AOPP患者氧化应激指标,减轻患者的微炎症状态,加快患者康复。
Objective:To investigate the effects of hemoperfusion(HP)combined with penehyclidine hydrochloride on oxidative stress and microinflammation in patients with severe organophosphorus pesticide poisoning(AOPP).Methods:A total of 70 AOPP patients treated with HP in the emergency department of Shishi General Hospital from February 2020 to December 2021 were selected and divided into two groups by random number table method,with 35 cases in each group.The control group was treated with HP,and the observation group was treated with HP combined with penehyclidine hydrochloride.The recovery,oxidative stress[lactate dehydrogenase(LDH),superoxide dismutase(SOD)]and microinflammation indicators[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]were compared between the two groups.Results:The coma time and CHE recovery time of observation group were shorter than those of control group(P<0.05).After 1 week of treatment,the LDH level of the two groups was lower than that before treatment,the observation group was lower than that of the control group,the SOD level of the two groups was higher than that before treatment,and the observation group was higher than that of the control group(P<0.05).After 1 week of treatment,the levels of IL-6 and TNF-αin the two groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05).Conclusion:HP combined with penehyclidine hydrochloride can improve the oxidative stress index of patients with AOPP,reduce the microinflammation state of patients,and accelerate the recovery of patients.
