摘要
目的建立标准化、规范化的药品不良事件(ADE)的管理体系,促进ADE规范管理。方法梳理ADE管理关键要素,构建组织和制订制度,对事件进行分类和分级管理;通过信息系统,按PDCA循环模式对整个事件流程进行环节控制,实时反馈,做到闭环管理;加强事件防控教育,制订奖惩措施,培养防控意识。利用医院信息系统收集2016年(实施前)和2017年至2021年(实施后)上报数据,采用问卷调查法收集上报知晓率、及时处理率等信息。结果ADE上报数量和知晓率均显著提高,医疗纠纷数量有所下降,环节管理流转效率(及时处理率)显著提高。结论基于信息化的ADE标准化管理模式可以规范、高效、便捷地对ADE进行闭环管理,增强医务人员ADE上报意识,降低临床医疗和用药安全风险。
ObjectiveTo construct a standardized management system of adverse drug events(ADE),and to promote the standardized management of ADE.MethodsThe key elements of ADE management were sort out,the organization was constructed,the system was formulated,and the ADE was classified and graded.Through the information system,the entire process of ADE was controlled by the PDCA cycle mode,with real-time feedback to achieve the closed-loop management.The prevention and control education of ADE was strengthened,the measures of reward and penalty were formulated,and the awareness of prevention and control was cultivated.The reported data in 2016(before the implementation)and from 2017 to 2021(after the implementation)were collected by the hospital information system.The awareness rate of reported ADE,timely rate of ADE processing and other data were collected by the questionnaire survey method.ResultsThe quantity of reported ADE and the awareness rate of reported ADE significantly increased,the incidence of medical disputes decreased,and the circulation efficiency of link management(timely rate of ADE processing)significantly increased.ConclusionThe standardized management mode of ADE based on informatization can realize the standardized,efficient and convenient closed-loop management for ADE,enhance the awareness of ADE reporting,decrease the clinical medical risk and medication risk.
