摘要
目的分析空气灌肠、经腹壁彩超与计算机断层扫描(CT)平扫对小儿肠套叠的诊断价值。方法选取2016年4月至2021年4月105例疑似肠套叠患儿作为研究对象,均行空气灌肠、经腹壁彩超与CT平扫检查。记录三种检查方法对肠套叠的诊断结果,并比较三种检查方法诊断肠套叠的灵敏度、特异度及准确度;分析三种检查方法诊断肠套叠的影像学特征;比较三种检查方法诊断肠套叠的耗时情况。结果105例疑似肠套叠患儿的临床最终诊断为肠套叠91例,非肠套叠14例。空气灌肠、经腹壁彩超与CT平扫诊断肠套叠的灵敏度、特异度均无明显差异(P>0.05);空气灌肠诊断肠套叠的准确度高于经腹壁彩超(P<0.05);空气灌肠与CT平扫、经腹壁彩超与CT平扫诊断肠套叠的准确度均无明显差异(P>0.05)。空气灌肠注入气体后可见充气结肠远端出现不同形态的高密度软组织,呈圆柱状、杯状;CT平扫显示有套筒征、腊肠征;经腹壁彩超中肠套叠区域的边界十分清晰,短轴切面呈现“靶环征”“同心圆”,同心圆直径<30 mm,超声显示“套筒征”,远端的血流信号分布十分丰富,而近段分布较分散。CT平扫诊断耗时均短于经腹壁彩超、空气灌肠,经腹壁彩超诊断耗时短于空气灌肠(P<0.05)。结论空气灌肠、经腹壁彩超与CT平扫对小儿肠套叠均有一定的诊断价值,其中空气灌肠的诊断准确度较高,CT平扫诊断所需耗时较短。
Objective To analyze the diagnostic value of air enema,transabdominal color doppler ultrasound and computed tomography(CT)scan in children with intussusception.Methods A total of 105 children with suspected intussusception from April 2016 to April 2021 were selected as the research objects.All children underwent air enema,transabdominal color doppler ultrasound and CT scan examinations.The diagnostic results of three examination methods for intussusception was recorded,and the sensitivity,specificity and accuracy of three examination methods for intussusception were compared.The imaging features of three examination methods in the diagnosis of intussusception were analyzed.The time consumption of three examination methods in the diagnosis of intussusception was compared.Results The final clinical diagnosis of 105 children with suspected intussusception was 91 cases of intussusception and 14 cases of non-intussusception.There were no significant differences in the sensitivity and specificity of air enema,transabdominal color doppler ultrasound and CT scan in the diagnosis of intussusception(P>0.05).The accuracy of air enema in the diagnosis of intussusception was higher than that of transabdominal color doppler ultrasound(P<0.05);there was no significant difference in the accuracy of air enema and CT scan,transabdominal color doppler ultrasound and CT scan in the diagnosis of intussusception(P>0.05).After gas injection of air enema,different forms of high-density soft tissue appeared in the distal end of the inflated colon,which were cylindrical and cup-shaped.CT scan showed sleeve sign and sausage sign.The boundary of intussusception area in transabdominal color doppler ultrasound was very clear,the short axis section showed"target ring""concentric circle",the diameter of concentric circle<30 mm,ultrasound showed"sleeve sign",the distribution of blood flow signals in the distal part was very rich,while the distribution in the proximal part was relatively scattered.The diagnostic time consumption of CT scan was sh
