摘要
目的:观察2型糖尿病气阴两虚证患者与健康人之间肠道菌群的组成结构及人血管生成素样蛋白4 (angiopoietinlike 4,ANGPTL4)、脂多糖(lipopolysaccharide,LPS)的差异性。方法:收集42例气阴两虚证患者为观察组,并以30例健康志愿者为对照组,对其粪便与血液标本进行检验,通过16S rDNA高通量测序的方法研究肠道菌群的组成特征。结果:(1)两组共有分类单元(operational taxonomic units,OTU)数为6 789,观察组独有OTU数为35 311,对照组独有OTU数为30 384。两组Beta多样性分析得出P=0.005,组间差异显著。(2)分类学组成显示,两组在门水平皆以厚壁菌门、拟杆菌门、放线菌门、变形菌门为主要优势菌群,占所有菌群的90%以上;LEfSe分析获得对照组的标志物种为大肠埃氏菌属-志贺氏菌属、拟杆菌科、拟杆菌属等。观察组的标志物种为硒单胞菌科、革兰氏阴性菌目、微小杆菌属等。(3)与对照组比较,观察组ANGPTL4下降,LPS水平明显上升(P﹤0.01)。(4)观察组拟杆菌属与FBG、HbA1C、ANGPTL4、LPS呈负相关,与ALT呈正相关(P﹤0.05);大肠埃氏菌属-志贺氏菌属与BMI、TG、ANGPTL4、LPS呈正相关(P﹤0.05)。结论:2型糖尿病气阴两虚证患者与健康人群在肠道菌群组成上存在显著差异,患者有明显肠道微生态失调的特征;患者的ANGPTL4、LPS水平显著异常,且与大肠埃希菌志贺氏菌属、拟杆菌属有显著相关性。
Objective:To study the differences of intestinal florae composition structure,human angiopoietin like protein 4(ANGPTL4)and lipopolysaccharide(LPS)between patients with type 2 diabetes mellitus(T2DM)with syndrome of deficiency of both qi and yin and healthy persons.Methods:42 cases of newly diagnosed T2DM patients with syndrome of deficiency of both qi and yin were collected and taken as observation group,and 30 cases of healthy volunteers were taken as control group.Their stool and blood samples were examined,and the composition characteristics of intestinal florae were studied by method of 16S rDNA high-throughput sequencing.Results:1.The number of operational taxonomic units(OTU)was 6789 in total in two groups,35311 OTU alone in observation group,and 30384 OTU alone in healthy control group.Beta diversity analysis in two groups showed that P=0.005 and the difference between two groups was significant.2.Taxonomic composition showed that firmicutes,bacteroidetes,actinobacteria and proteobacteria were the main dominant florae at phylum level in both groups,and accounting for more than 90%of all the florae.The marker species of control group obtained by LEfSe analysis were escherichiashigella,bacteroidaceae,bacteroides,etc.The marker species of observation group were selenomonadales,negativicutes,exiguobacterium,and others.3.Compared with control group,the level of ANGPTL4 decreased in observation group,and the level of LPS increased significantly(P﹤0.01).4.Bacteroides of observation group were negatively correlated with FBG,HbA1C,ANGPTL4 and LPS,while positively correlated with ALT(P﹤0.05).Escherichia-shigella were positively correlated with BMI,TG,ANGPTL4 and LPS(P﹤0.05).Conclusion:The composition of intestinal microflorae in patients with T2DM with syndrome of deficiency of both qi and yin are significantly different from healthy persons,and the characteristics of intestinal microecological imbalance of patients are obvious.The levels of ANGPTL4 and LPS of patients are significantly abnormal and significa
