血清细胞角蛋白19片段联合神经特异性烯醇化酶、癌胚抗原检测在肺癌辅助诊断中的价值 被引量:7

摘要 目的探讨血清细胞角蛋白19片段(CYFRA21-1)联合神经特异性烯醇化酶(NSE)与癌胚抗原(CEA)检测在肺癌辅助诊断中的应用价值。方法选取2019年6月至2021年5月杭州师范大学附属医院与安徽医科大学第一附属医院收治的肺癌患者共57例作为研究对象,同时纳入同期入院的57例肺良性疾病患者作为对照组,电化学发光法检测各组患者血清CYFRA21-1、NSE和CEA水平,受试者工作特征(ROC)曲线分析3种血清肿瘤标志物单独及联合检测在肺癌辅助诊断中的应用价值。结果肺癌组和肺良性疾病组CYFRA21-1、NSE和CEA血清学中位水平分别为3.71(2.31,9.61)ng/ml比1.99(2.50,3.58)ng/ml、17.78(13.65,26.60)ng/ml比15.55(11.67,18.45)ng/ml和4.23(2.06,10.91)ng/ml比2.33(1.62,3.53)ng/ml,差异有统计学意义(Z=-3.006、-2.474、-3.500,P均<0.05);CYFRA21-1、NSE和CEA在肺癌组与肺良性疾病组阳性检出率分别为53%比33%、67%比33%、和42%比16%,差异亦均有统计学意义(χ^(2)=9.596、12.667和4.331,P均<0.05)。ROC曲线分析显示,3种血清肿瘤标志物联合检测诊断肺癌的ROC曲线下面积(AUC=0.749)高于CEA、NSE和CYFRA21-1单项检测诊断肺癌的ROC曲线下面积(AUC=0.690、0.634和0.663)。结论血清CYFRA21-1、NSE与CEA均可作为肺癌血清学诊断指标,且CYFRA21-1联合NSE、CEA检测能明显提高肺癌阳性检出率,有利于肺癌的早期辅助诊断。 Objective To explore the value of serum tumor markers including cytokeratin 19 fragment(CYFRA21-1),neuron specific enolase(NSE)and carcinoembryonic antigen(CEA).Methods A total of 57 patients with lung cancer(lung cancer group)who were diagnosed and treated in the Affiliated Hospital of Hangzhou normal University and the First Affiliated Hospital of Anhui Medical University between June 2019 and May 2021 were selected as the subjects,and 57 patients with benign lung lesions(benign lesion group)who were diagnosed and treated in the hospital during the same period were also enrolled in the study.The serum CYFRA21-1,NSE and CEA levels in each group were detected by electro chemiluminescence immunoassay.The receiver operating characteristic(ROC)curve was used to analyze the value of CYFRA21-1,NSE and CEA levels in the diagnosis of lung cancer.Results The serum median level of CYFRA21-1,NSE and CEA in lung cancer group and benign lesion group were 3.71(2.31,9.61)ng/ml vs.1.99(2.50,3.58)ng/ml,17.78(13.65,26.60)ng/ml vs.15.55(11.67,18.45)ng/ml and 4.23(2.06,10.91)ng/ml vs.2.33(1.62,3.53)ng/ml respectively,and the differences were statistically significant(Z=-3.006,-2.474,-3.500,all P<0.05);the positive rates of CYFRA21-1,NSE and CEA were significantly higher in the lung cancer group than in the benign lesion group(53%vs.33%,67%vs.33%and 42%vs.16%),and the differences were statistically significant(χ^(2)=9.596,12.667,4.331,all P<0.05).ROC curve analysis showed that area under the curve(AUC=0.749)of the combined detection of serum CYFRA21-1,NSE and CEA in the diagnosis of lung cancer were significantly higher than those of the individual indicators CEA,NSE and CYFRA21-1(AUC=0.690,0.634,0.663,all P<0.05).Conclusion The serum CYFRA21-1,NSE and CEA are valuable tumor markers for diagnosis of lung cancer.CYFRA21-1 combined with NSE and CEA detection can significantly improve the positive rate of lung cancer,is beneficial for the early auxiliary diagnosis of lung cancer.
