超声在早期强直性脊柱炎骶髂关节病变治疗前后的对比应用

Comparative application of ultrasound before and after treatment of sacroiliac joint lesions in early ankylosing spondylitis

摘要 目的 探讨超声在早期强直性脊柱炎(AS)患者治疗前后的骶髂关节病变中的对比应用。方法 根据纽约标准诊断强直性脊柱炎,将30例活动性强直性脊柱炎患者治疗前后进行对照研究。所有受检者治疗前后均探查骶髂关节的血流信号以及骶髂关节的骨侵蚀情况,血流信息包括收缩期峰值流速(PSV)、收缩期阻力指数(RI)及收缩期搏动指数(PI),并根据血流量进行分级。结果 强直性脊柱炎患者在治疗前后骶髂关节中血流信息的变化显著,治疗后血流信号中的RI及PI明显增高,血流量分级减低(P<0.05)。骨侵蚀在治疗3个月后变化不显著(P>0.05)。结论 超声评价早期强直性脊柱炎患者的骶髂关节骨侵蚀情况及血流信号的变化,可以有效地为临床治疗提供重要的参考信息。 Objective To explore the comparative application of ultrasound in sacroiliac joint lesions of patients with early ankylosing spondylitis before and after treatment.Methods According to the New York standard, 30 patients with ankylosing spondylitis were studied before and after treatment.The blood flow signal of sacroiliac joint and the bone erosion of sacroiliac joint were detected before and after treatment in all subjects.The blood flow information included measuring the resistance index and pulsatility index, and grading according to the blood flow.Results The changes of blood flow in sacroiliac joint of patients with ankylosing spondylitis before and after treatment were significant.After treatment, the resistance index and pulsation index of blood flow signal were significantly increased, and the blood flow grade was decreased(P<0.05).There was no significant change in bone erosion after three months of treatment(P>0.05).Conclusion Ultrasonic evaluation of sacroiliac joint bone erosion and blood flow signal changes in patients with early ankylosing spondylitis can effectively provide important reference information for clinical treatment.
