摘要
目的 观察关节镜下髌骨去神经化联合手法髌骨内推治疗髌股关节炎的临床疗效。方法 将60例膝关节髌股关节炎患者随机分为治疗组和对照组,治疗组30例行关节镜下髌骨去神经化及手法髌骨内推治疗,对照组30例行关节镜下髌骨去神经化治疗。手法髌骨内推治疗自术后第3天开始,1个月为1个疗程,持续5个疗程。治疗前、治疗6个月后采用美国西部Ontario与McMaster大学骨关节炎指数(WOMAC)评分以及髌股关节功能(Kujala)评分进行评价。结果 对照组和治疗组在治疗半年后WOMAC评分均较治疗前有所下降,Kujala评分治疗后明显高于治疗前,均有统计学差异(P<0.05)。所有患者均未出现重要血管神经损伤以及髌骨坏死等并发症。结论 关节镜下髌骨去神经化联合手法髌骨内推治疗可有效缓解髌股关节炎引起的膝前疼痛,改善膝关节功能。
Objective To observe the effect of the treatment of patellofemoral arthritis under arthroscope by denervation of patella combined with massage of patella.Methods Sixty patients with patellofemoral arthritis of the knee were randomly divided into two groups, 30 cases were treated with denervation of patella only under arthroscope(control group),30 cases were treated with denervation of patella under arthroscope and massage of patella(treatment group).Manual kneecap massage was started from the 3 rd postoperative day, 1 month as a course of treatment, all patients in the treatment group continued treatment for 5 courses.The osteoarthritis index of Ontario and McMaster universities in western United States(WOMAC) and patellofemoral function score(Kujala) were used for evaluation before and 6 months after treatment.Results The WOMAC score of the control group and the treatment group was lower than that of the control group after six months of treatment, and the Kujala score was significantly higher than that before treatment, the difference was statistically significant(P<0.05).No significant vascular and nerve injury and patellar necrosis were observed in all patients.Conclusion The combination of denervation of patella under arthroscope and massage of patella can effectively relieve the anterior knee pain caused by patellofemoral arthritis and improve the knee joint function.
